Several treatments for menopausal syndrome

(2) Drug types and preparations ① Estrogens Natural steroidal estrogen preparations such as estradiol, estradiol valerate, conjugated estrogens, estriol, estrone; some synthetic estrogens such as ethinyl estradiol, ethinyl estradiol trimethyl ether; synthetic estrogens such as nialstradiol. ②Progestin counteracts the effect of estrogen in promoting endometrial growth. There are three categories: 19-demethyltestosterone derivatives (such as norethindrone), 17-hydroxyprogesterone derivatives (such as medroxyprogesterone), and natural progesterone (such as micronized progesterone). ③ The decomposition products of the estrogen, progestogen and androgen combination drug tibolone after entering the body have progestogen, androgen and weak estrogen activities, and do not stimulate endometrial hyperplasia.

(3) The routes of administration include oral administration, vaginal administration, and skin administration, which can be selected according to the condition and patient's wishes.

(4) Commonly used regimens ① The continuous sequential method uses a 28-day treatment cycle, with estrogen being used continuously and progesterone being used from day 15 to day 28 of the cycle. No breaks between cycles. This regimen is suitable for women within 3 to 5 years of menopause. ② The sequential method uses 28 days as a treatment cycle. Estrogen is given every day from the 1st to the 21st day, progesterone is given from the 11th to the 21st day, and the medication is stopped from the 22nd to the 28th day. Withdrawal bleeding may occur after the progestin medication is stopped. This program is suitable for women in perimenopause and premature ovarian failure. ③ In continuous combined therapy, both estrogen and progestin are given daily, and the chance of withdrawal bleeding is low. Suitable for women who have been postmenopausal for many years. ④ Estrogen therapy alone is suitable for women with ovarian dysfunction after hysterectomy or congenital absence of uterus. ⑤ Single progestin therapy is suitable for women in the menopausal transition period or postmenopausal women with severe perimenopausal symptoms and contraindications to estrogen. ⑥ Adding a small amount of androgen to HRT can improve mood and sexual desire.

(5) The optimal dose of HRT is the minimum effective dose for clinical effect, which can achieve the therapeutic purpose, prevent endometrial hyperplasia, and keep the E2 content in the blood at the early level of premenopausal follicles.

(6) Duration of medication ① Short-term medication: HRT lasts for less than 5 years and is called short-term medication. The main purpose is to relieve perimenopausal symptoms, which usually take effect within 1 month and achieve stable relief within 4 months. ② Long-term medication is used to prevent and treat osteoporosis, lasting at least 3 to 5 years.

(7) Side effects and risks: uterine bleeding, sex hormone side effects, progesterone side effects, endometrial cancer, and breast cancer.
