The vagina is an elastic muscular lumen, with the upper end wider than the lower end, and the vaginal mucosa has many transverse folds. The front and back walls are usually tightly attached, so once a foreign body enters the vagina, it is difficult for it to fall off on its own.

1. Remove foreign matter

The method of removing a foreign body varies slightly depending on the patient's age and the size, location, and shape of the foreign body.

1. Removal through the vagina: Older children can insert their fingers into the vagina to hook out the foreign body, or use a small speculum to pick up the foreign body with forceps under direct vision. Adults can directly pick up the foreign body under direct vision with the speculum. When removing sharp foreign objects, the long axis of the foreign object should be parallel to the longitudinal axis of the vagina, and the sharp end of the foreign object should be facing the vaginal opening to prevent damage to the mucosal tissue.

2. Rectal examination and displacement method: Young children can use hemostatic forceps or a small scraper to reach into the vagina under the guidance of the rectal examination fingers to push out the foreign body.

3. Nasal endoscope: Use a nasal endoscope to dilate the child's vagina and use forceps to remove foreign objects. If the child cannot cooperate, intravenous ketamine anesthesia can be performed.

4. Remove foreign bodies under direct vision of the hysteroscope: Coat the hysteroscope with lubricant and slowly insert it into the vagina. Then use the left thumb and index finger to hold the cotton ball to compress the vulva to prevent the outflow of fluid and make the vagina swell. Under direct vision of the hysteroscope, comprehensively observe the pathological changes and perform necessary treatment according to the condition, such as removing foreign bodies, taking biopsy tissues, flushing and applying medicines, etc.

5. Microcolposcope: It uses normal saline as the uterine distension fluid, and can peek into the entire cervix and vagina to detect small foreign objects such as colored crayons, and can be treated under direct vision. It is used for the diagnosis and treatment of vaginal diseases in prepubertal children. It is simple to operate, causes little damage, and is easy to accept.

6. Vaginal irrigation: A catheter can be inserted into the vagina and the vagina can be repeatedly flushed with 40% comfrey oil under pressure. Sometimes small foreign objects such as sand and wheat grains can be flushed out by the flushing fluid, which also has an anti-inflammatory effect.

2. Local treatment

After the foreign body is removed, routine treatment for vaginitis should be given, including vaginal flushing with 0.5% to 1% acetic acid solution, or 1/5000 potassium permanganate solution, or 5% iodine swab, or local application of erythromycin ointment.

3. Vaginal surgical repair

Anyone with urinary fistula or fecal fistula requires surgical repair.

4. Vaginal adhesion separation

Foreign objects left in the vagina for a long time may cause vaginal adhesions or partial vaginal occlusion, which should be separated surgically.

5. Laparotomy

Among the complications caused by vaginal foreign bodies, most can be resolved by vaginal surgery, and very few require laparotomy, such as pelvic abscess, broad ligament abscess, ureterovaginal fistula, etc. caused by vaginal foreign bodies, or foreign bodies left at the top of the vagina during a previous laparotomy.

6. Treatment of Toxic Shock Syndrome

Intensive supportive treatment is the key to successful rescue.
