Diagnosis and treatment of pregnancy complicated with bilateral lower extremity venous thrombosis

Pregnancy complicated by bilateral lower limb venous thrombosis is a disease that is relatively difficult to cure. So what are the diagnosis and treatment methods for pregnancy complicated by bilateral lower limb venous thrombosis? Pregnancy complicated by bilateral lower limb venous thrombosis is better than the slow blood flow in the lower limb veins caused by various reasons. The blood is generally in a state of hypercoagulability. In severe cases, it can cause disease lesions and cause great harm to patients.

diagnosis:

1. It is more common in patients who are postpartum, have pelvic surgery, trauma, advanced cancer, coma or long-term bedridden.

2. The onset is relatively acute, with swelling, stiffness and pain in the affected limb, which worsens after activity, and is often accompanied by fever and rapid pulse.

3. Tenderness at the site of thrombus, cord-like objects can be felt along the blood vessels in the distal limbs or the entire limb is swollen, the skin becomes bluish purple, the skin temperature decreases, the pulsation of the posterior tibial artery of the dorsal foot is weakened or disappears, or venous gangrene occurs; when the thrombus extends to the inferior vena cava, both lower limbs, buttocks, lower abdomen and external genitalia are obviously edematous. When thrombosis occurs in the calf muscle venous plexus, Homans sign and Neuhof sign are positive.

4. In the later stage, the thrombus is absorbed and organized, often leaving the veins behind with incomplete function, resulting in superficial varicose veins with pigmentation, ulcers and swelling, etc., which is called post-deep vein thrombosis syndrome. It can be divided into: ① peripheral type with blood backflow as the main feature; ② central type with blood reflux disorder as the main feature; ③ mixed type with both blood backflow and reflux disorder.

5 Treatment:

1. Surgery

Surgical thrombectomy is an effective method to eliminate blood clots and can quickly relieve venous obstruction.

2. Medication

Although intravenous heparin can immediately play an anticoagulant role and thus exert a thrombolytic effect by mobilizing endogenous cytokines, it seems to be more passive compared with direct thrombolytic agents. Since streptokinase is antigenic and often causes pyrogenic reactions after intravenous administration, the use of urokinase is recommended.
