Antibodies targeting lipids are not specific because they are not directly targeting Treponema pallidum. In addition to syphilis, other diseases and changes in physiological conditions may also produce low-titer anti-lipid antibodies in the body. When diagnosing syphilis, the syphilis serological examination is performed to detect these two types of antibodies. The RPR test mentioned above is an experiment to detect lipid antibodies; while TPHA is an experiment to directly detect Treponema pallidum.

Because RPR is a test for lipid antibodies, not for direct detection of anti-syphilis Treponema antibodies, it is non-specific. Any disease that can lead to the production of lipid antibodies can make RPR positive. In addition to syphilis, upper respiratory tract infection, pneumonia, active pulmonary tuberculosis, rheumatic heart disease, subacute bacterial endocarditis, infectious hepatitis, cirrhosis, chronic nephritis, leptospirosis, leprosy, malaria, rheumatoid arthritis, systemic lupus erythematosus and heroin addiction can all lead to RPR positive.
