Uterine bilateral adnexectomy

Uterine bilateral adnexectomy

The uterine adnexa are the female reproductive organs; the fallopian tubes and ovaries on the left and right sides of the female uterus. Collectively referred to as "uterine adnexa", or "adnexa" for short. The fallopian tube is divided into the narrow part, the ampulla of the fallopian tube, and the fimbria of the fallopian tube. If there are any abnormalities in the uterine adnexa, a resection surgery is necessary. Let me introduce you to some relevant knowledge below.

Uterine oophorectomy

1. Indications:

1. Uterine fibroids, functional uterine bleeding, adenomyoma, normal cervical examination, and the patient requests to retain the cervix.

2. Those who need to have their uterus removed for various reasons but have difficulty removing the cervix.

Preoperative preparation: Cervical smear and diagnostic curettage are performed before surgery to rule out malignant tumors of the cervix and uterine body.

2. Surgical steps:

After successful anesthesia, the patient takes the supine position, and after routine abdominal disinfection and draping, a midline longitudinal incision of about 10 cm is made in the lower abdomen. The skin, subcutaneous tissue, fascia, and anterior rectus sheath are cut in sequence, the rectus abdominis is bluntly separated, the peritoneum is opened, and the abdominal cavity is exposed.

1. Treatment of the round ligament: Expose the left round ligament, clamp the left round ligament with two middle-curved forceps, cut it between the forceps, and suture the stump with No. 7 silk thread. Treat the right round ligament in the same way.

2. Dealing with the appendages: Expose the left appendages, clamp the left fallopian tube isthmus and the left ovarian proper ligament with a middle-curved forceps, with the tip of the forceps reaching the avascular area of the broad ligament and cutting it off. The stump was double-sutured with No. 7 silk thread. Treat the right side attachment in the same way.

3. Push the bladder down: cut the bladder peritoneum fold and push the bladder down to below the external cervical opening.

4. Handle the uterine blood vessels: clamp and cut off the loose tissues around the left uterus, expose the left uterine blood vessels, and clamp and cut them. The stump was double sutured with No. 7 silk thread. Treat the right uterine vessels in the same way.

5. Treatment of the uterosacral ligament: Expose the uterosacral ligament, clamp the cervix close to the left uterosacral ligament. The stump was sutured with No. 7 silk thread. Treat the right uterosacral ligament in the same way.

3. After undergoing bilateral hysterectomy, you should pay attention to the following matters to avoid adverse consequences.

1. After the operation, the lower abdomen may be slightly swollen, which may be caused by muscle relaxation. A physical therapist can teach you exercises to strengthen the muscles in this area. But it may take weeks or longer for the team to return to normal.

2. After the operation, while the internal wounds are healing, the vagina will discharge a small amount of excrement or have slight bleeding, just like menstruation, and this situation will last for several weeks. During this time, you need to use sanitary napkins to avoid staining your clothes, but do not use tampons as they may cause bacterial infection.

3. Make sure to get more rest and let others help you with housework and shopping. You can do some light activities within six weeks after surgery, but no strenuous exercise.

4. After a major operation, due to the effect of anesthesia, it will take several weeks for the patient's concentration and reaction to return to normal. If you are driving, you should get your doctor's approval before resuming driving.

5. After about six to eight weeks, seek medical advice to see if you can return to work.

Finally, I would like to suggest that everyone should pay attention to taking Chinese medicine for conditioning after bilateral hysterectomy to prevent recurrence. At the same time, you can take estrogens such as progesterone and progesterone, which can effectively and safely maintain female characteristics. The frequency of sexual intercourse should be reduced and regular. There is more knowledge, please follow the editor at any time!
