Generally speaking, the uterus will shrink back to its original state about 10 days after delivery, and will fully recover after 4-6 weeks. However, if after delivery your uterus feels very large, you keep discharging lochia mixed with blood, and you have abdominal pain, you should suspect that you have uterine involution.

The process by which the pregnant uterus gradually returns to its non-pregnant state after the placenta is delivered is called uterine involution. The causes of incomplete uterine involution include: part of the egg membrane or placenta retained in the uterus; early rupture of the amniotic fluid; twin pregnancy; the bladder or rectum is filled with excrement due to difficulty in urination or defecation, etc. In addition, uterine fibroids are also one of the causes of incomplete uterine involution.

Uterine contractants or hemostatic drugs should be used during treatment. If there is a lot of bleeding and bacterial infection is a concern, antibiotics can also be used in combination.

On the first day after delivery, the external cervical os rises to the level of the ischial spine, causing the fundus of the uterus to rise slightly to the level of the navel. It then descends 1 to 2 cm every day, and on the 10th day after delivery, the uterus descends into the pelvic cavity.

The speed of uterine involution is related to the mother's age, number of births, general health condition, length of labor, method of delivery, and whether or not she is breastfeeding.

Residues of placenta or fetal membranes, endometritis, pelvic inflammatory disease, retroflexed uterus, uterine fibroids, etc. will hinder uterine involution.
