Because it is confined to a part of the uterus, it often causes the uterus to enlarge irregularly, resembling uterine fibroids. The cross-section shows that the proliferating muscle tissue also has a whirlpool-like structure similar to that of a tumor, but there is no capsule-like tissue that separates the normal muscle fibers on all four sides as in a tumor. There is a softening area in the center of the lesion, and occasionally scattered small cavities containing a small amount of old blood can be seen. The endometrial glands seen under microscopy are the same as those of the endometrium, and are surrounded by endometrial stroma on all sides. The ectopic endometrium changes with the menstrual cycle, but the changes in the secretory phase are not obvious, indicating that the ectopic endometrial glands are less affected by progesterone.
