Spotting a few days before your period, if it only happens once, could be implantation bleeding from the fertilized egg after pregnancy, or occasional bleeding caused by insufficient progesterone during the luteal phase. However, if spotting occurs repeatedly a few days before your period, it could be due to insufficient progesterone during the luteal phase, or endometriosis. Both of these reasons increase the chances of infertility. In a few cases, the cause is organic (meaning actual lesions) such as polyps and myos in the uterus.

In addition, women with cervical polyps, ectropion or erosion, and those who have just had an IUD installed are prone to bright red bleeding immediately after sexual intercourse. Some normal women may also have brown punctate bleeding due to endometrial instability caused by the impact of uterine blood circulation during sexual intercourse. But the latter reasons do not usually occur only a few days before menstruation.

People with progesterone deficiency in the luteal phase tend to have shorter menstrual cycles, which may be reflected in a shorter high temperature period (luteal phase) of basal body temperature (less than 11 days) and lower blood progesterone levels in the middle of the high temperature period. Some infertility physicians will take endometrial samples in the middle of the high temperature period. If the endometrial hormone changes seen by the pathology department under the microscope lag behind the high temperature period by more than 2 days, it means progesterone deficiency. If you suspect that you have "luteal phase progesterone deficiency", you can supplement progesterone to optimize the cycle when preparing for pregnancy.
