Treatment of ovarian endometriosis

Treatment of ovarian endometriosis

Endometriosis is a gynecological disease that many women suffer from. The consequences it brings to us are different due to the different sites of the disease. Among them, ovarian endometriosis is a more serious one. It can cause infertility in women to a large extent. Therefore, if such a disease occurs, we must treat it in time to avoid serious impact on our lives. Let’s take a look at the treatment methods of ovarian endometriosis.

Causes

There are three theories about the occurrence of endometriosis: implantation theory, metaplasia theory and viviparous epithelial origin theory. The implantation theory is the most popular theory in the study of ovarian endometriomas. Implantation theory: Sampsan first proposed this theory in 1921. This theory holds that ectopic endometrium originates from endometrial tissue that migrates to sites outside the uterine cavity and then implants and grows. Common transmission routes include retrograde menstruation, iatrogenic implantation, lymphatic spread and vascular dissemination.

Clinical manifestations

1. Dysmenorrhea

Dysmenorrhea is the most obvious symptom of endometriosis, but it is also the most easily overlooked precursor.

2. Pain during sexual intercourse

This is also an obvious signal, which may be accompanied by punctate bleeding. The symptoms of ovarian chocolate cyst are not very obvious.

3. Infertility

Among infertile women, endometriosis accounts for a high proportion of infertility.

Treatment of ovarian endometriosis

It can be treated with medication or (and) surgery (conservative or radical). So far, there is no ideal cure except radical surgery. Both drug treatment and conservative surgery have a very high recurrence rate. Therefore, the selection should be based on the patient's age, symptoms, signs, extent of lesions, and fertility requirements, emphasizing individualized treatment. Mild lesions with mild or no symptoms are treated expectantly; mild patients with fertility desires will receive drug treatment after a clear diagnosis, and those with severe conditions will undergo fertility-preserving surgery; young severe patients with no fertility desires can undergo ovarian function-preserving surgery supplemented by drug treatment; patients with severe symptoms and lesions and no fertility desires can undergo radical surgery.

There are two surgical methods: open surgery and laparoscopic surgery. Laparoscopic surgery is the best treatment for this disease. Currently, laparoscopic diagnosis, surgery + medication are considered the gold standard for the treatment of endometriosis. In hospitals with the necessary conditions, laparoscopic surgery should be the preferred treatment for endometriosis. However, the choice and effect of laparoscopic surgery depend to a large extent on the equipment, instruments and the experience and skills of the surgeon. Open surgery can be used for surgeries that are not suitable for laparoscopy or are very complicated, such as those with severe adhesions and a history of multiple surgeries.

There are many treatments for ovarian endometriosis. We should not always make blind choices during treatment. We should choose the treatment method based on our own condition. Usually there are two types of treatments: drug treatment and surgical treatment. When choosing, we must listen to the doctor's opinions and suggestions and go to a regular specialist hospital for treatment to get the best recovery.
