Among the antihypertensive drugs, can pril, sartan and lolamine be used together?

A friend told Huazi that he had been using two antihypertensive drugs, Sartan and Lol, in combination to control blood pressure, and the effect was good. But he heard someone say that Sartan drugs and Lol drugs cannot be used together, so he asked Huazi if this statement was true?

Hua Zi said that sartan drugs and loral drugs can be used together, but it is not a mainstream antihypertensive drug combination. Because the two have overlapping pharmacological mechanisms, the combined use of drugs may not achieve the "1+1>2" antihypertensive effect. However, for hypertensive patients with heart disease, such a combination of drugs is suitable for protecting the heart.

1. Prils and Sartans

Among antihypertensive drugs, prils and sartans belong to the same class of drugs. Both can inhibit the "renin-angiotensin-aldosterone" system (RAAS) and exert a blood pressure-lowering effect. If RAAS is regarded as a "rope", prils are the first half of the rope that is cut, and sartans are the second half of the rope that is cut.

RAAS is also a major risk factor for heart damage. Pril and sartan can prevent the proliferation of fibroblasts and apoptosis of endothelial cells, reverse myocardial hypertrophy, ventricular remodeling, alleviate heart failure, and protect the kidneys.

It should be noted that pril and sartan can replace each other, but the two cannot be used together . Using the two together will not only not increase cardiovascular protection, but will make adverse reactions more likely to occur.

2. Lolamine drugs

Lolamine drugs are beta-blockers that can block the beta-1 receptors on the heart , inhibit myocardial excitability, weaken myocardial contractility, lower heart rate, reduce cardiac output, lower blood pressure, and reduce cardiac oxygen consumption.

Lolamine drugs can also block beta receptors on nerve synapses, reduce sympathetic nerve activity, reduce renin secretion, and inhibit the "renin-angiotensin-aldosterone" system (RAAS) . They are suitable for the treatment of hypertension, coronary heart disease, arrhythmia and other diseases.

In other words, Lol drugs can lower blood pressure in many ways. The common triggering factor of hypertension in young and middle-aged people is sympathetic nerve overexcitation, which is manifested as an increased resting heart rate (over 80 per minute). Lol drugs are more effective.

3. Combination of Priligy or Sartan with Lolamine

Not a commonly used antihypertensive combination : About 70% of hypertensive patients cannot control their blood pressure well with a single antihypertensive drug, and need to use two or more antihypertensive drugs in combination. In the combination of antihypertensive drugs, pril or sartan and lol are not a commonly used drug combination .

The reason is that both types of drugs can inhibit RAAS, and their pharmacological mechanisms overlap , so they cannot achieve the "1+1>2" drug combination effect. The commonly used regimens are "dipine + pril or sartan" or "dipine + lolamine", which can complement each other in pharmacological mechanisms and produce a "1+1>2" antihypertensive effect.

A good partner for treating heart disease : But for people with coronary heart disease, myocardial infarction, heart failure and other heart diseases, the combination of pril or sartan and lol is a "good partner". Pril or sartan can reverse myocardial hypertrophy, ventricular remodeling, and prevent atrial fibrillation; lol drugs can reduce cardiac oxygen consumption and prevent the occurrence of angina pectoris.

Therefore , for people with heart disease, especially those with heart disease and hypertension, the combination of the two types of drugs can produce a good synergistic effect , reduce acute cardiovascular events, and reduce the morbidity and mortality of heart disease patients. It is a commonly used drug combination regimen.

In summary, pril, sartan and lol are all commonly used antihypertensive drugs, among which pril and sartan have more overlapping pharmacological mechanisms and cannot be used together. Pril or sartan and lol have some overlapping pharmacological mechanisms and are not commonly used antihypertensive drug treatment options, but for people with heart disease, the combination of the two types of drugs can produce a good synergistic effect and is a common treatment option.
