Rubbing the glans made me urinate

Many people suddenly urinate when they rub the glans. This depends on whether it is nutritious, because beverages often cause local inflammation and infection due to pressure or stimulation, which are the main factors causing sudden urine leakage. You can go to the hospital for testing to determine the cause. It does not rule out genetic, physiological factors or inflammatory infection, which lead to the symptoms of enuresis.

Pathological etiology

There are many reasons that may lead to the occurrence of enuresis, and it may even be the result of multiple factors working together.

1. Genetic factors: Enuresis has a clear genetic tendency, and the genetic gene is located on chromosome 22q11. Statistics show that the incidence of enuresis in offspring of parents with enuresis, unilateral enuresis, and no enuresis are 77%, 44%, and 15%, respectively.

2. Developmental delay If the development of the brain's arousal center is delayed, the urination pattern completed by low-level central reflexes in infancy will be maintained. Such children often have other developmental delays. Children's thalamus and pituitary glands are underdeveloped, and the secretion of antidiuretic hormone decreases at night, resulting in increased urine volume at night. This mismatch between the reduced functional bladder capacity and the urine volume at night is an important cause of enuresis.

3. Urodynamic factors Urodynamic examinations during sleep found that 1/3 to 1/2 of the children had extremely active bladders, with frequent and large-amplitude spontaneous contractions. This unstable bladder can further reduce the functional bladder capacity of the children.

4. Secondary to urinary tract infection or obstruction such as posterior urethral valve disease and annular stenosis.

5. Abnormal neurological function.

6. Psychological factors.

Disease diagnosis

1. Urinary incontinence refers to the loss of control over urine, which flows out without the will, usually with no obvious difference between day and night.

2. Ectopic ureter When a woman has an ectopic ureter, in addition to normal urination through the urethra, urine leakage may also be found in other parts of the body outside the urethra. Examinations such as IVU may show deformities in the urinary system.

Inspection method

Laboratory tests:

Urinalysis was normal and urine culture showed no bacterial growth. The content of arginine vasopressin (AVP) secreted by the neurohypophysis in urine was measured during the day and night. Normally, urine volume increases at night compared to during the day, and its antidiuretic effect reduces urine volume at night. In patients with enuresis, due to delayed development of the thalamus and pituitary gland, nocturnal AVP secretion does not increase, resulting in increased nocturnal urine volume.

Other auxiliary examinations:

1. Ultrasound, IVU and cystourethrography during micturition are used to understand the condition of the kidneys, ureters and bladder. Generally, no abnormalities are found. There was no congenital spina bifida and meningocele on plain radiographs.

2. Urodynamic examination Urodynamic examination should be performed on all patients suspected of having neurological diseases, daytime urinary incontinence without pathological changes, adolescent nocturnal enuresis that is not responsive to conventional treatment, incontinence of both urination and defecation, persistent dysuria after infection control, recurrent urinary tract infections despite continuous antibiotics, and patients with bladder trabeculation or sphincter spasm shown on cystography during micturition.
