Is the success rate of artificial insemination high?

1. Indications

Male factors: sexual dysfunction, mild asthenospermia; ovulation disorders; unexplained infertility; endometriosis (mild to moderate); cervical infertility.

2. Contraindications

Acute and chronic systemic diseases and genital tract inflammation; sperm and egg combination disorder caused by fallopian tube factors on the female side; the female side suffers from genetic diseases, serious physical diseases, mental and psychological disorders; there is a history of birth of babies with congenital defects and it is confirmed that the cause is the female side; the female side is exposed to teratogenic amounts of radiation, poisons, and drugs and is in the effect period; the female side has bad habits such as alcoholism and drug abuse.

3. Semen conditions for IUI in men with asthenospermia

Sperm density ≥ 15 million/ml, forward-moving sperm (a+b) ≥ 15%; conditions for IUI after semen processing: upstream a-grade sperm ≥ 70%, sperm ≥ 20/HPF, at least one fallopian tube is unobstructed.

4. Diagnosis and treatment process

(1) Female preparation ① Natural cycle IUI: Women with regular menstruation can undergo natural cycle IUI. Patients begin monitoring the growth of follicles on the 8th to 10th day of the menstrual cycle. ② Ovulation induction cycle IUI: For those with ovulation disorders, ovulation induction is required. The patient returns for a checkup on the 2nd to 3rd day of the menstrual cycle to rule out pregnancy and ovarian cysts by ultrasound examination, and ovulation-inducing drugs can be given. ③ When the dominant follicle reaches 1.4 cm, establish an IUIB ultrasound monitoring form, fill in the monitoring form completely, and clearly state the IUI indications.

(2) Review the “three certificates”. Tell the patient that medical records do not need to be retrieved during the B-ultrasound in this cycle. ④IUI timing: IUI is performed one day after the LH peak; IUI on the ovulation day; HCG is injected when at least one follicle reaches 18-20mm in diameter, and IUI is performed 24-36 hours after HCG injection. ⑤ If there are >3 dominant follicles in the ovulation induction cycle, IUI in this cycle will be abandoned. ⑥ Luteal support: Oral progesterone can be given for 14 days starting from the third day after ovulation, and urine and blood HCG should be checked 16 days after the operation to confirm whether the pregnancy is confirmed.

(2) The man prepares to masturbate and ejaculate once on the 8th day of the woman’s menstrual cycle. On the day of IUI, the man masturbates to collect semen, and the number of forward-moving sperm injected into the husband's semen for artificial insemination should be more than 1 million.

5. Frequency of IUI per cycle

IUI can be performed 1 to 2 times per cycle. A meta-analysis of more than 1,000 IUI cycles showed that two IUIs could slightly improve fertility compared with a single IUI, but the difference was not statistically significant (14.9% vs. 11.4%).

6. Number of IUI cycles

The pregnancy rate is highest after 3 to 4 cycles of IUI. After 3 to 6 cycles of IUI, the cumulative pregnancy rate enters a plateau period.
