Ovarian hematoma is a relatively serious disease that may endanger life safety. If the ovary is severely ruptured and bleeding and repair is difficult, oophorectomy is performed. In any case, the ovarian function should be preserved as much as possible. If the abdominal pain disappears, conservative treatment can be considered, giving hemostatics and antibiotics. This is the best.

What causes ovarian hematoma?

The theca layer of the mature ovarian follicles has a rich vascular network. If it ruptures due to some inducement, bleeding may occur around it or blood may flow into the follicular cavity. During the corpus luteum angiogenesis stage, blood vessels grow actively and penetrate into the granulosa layer until they reach the edge of the corpus luteum cavity and bleed. Generally, the amount of bleeding is very small, and pathological ovarian hematoma can only be formed when the bleeding is severe. Therefore, depending on the time of formation of ovarian hematoma, it can be divided into follicular hematoma and corpus luteum hematoma.

Follicular ovarian hematoma: The hematoma is generally small and often confined to the follicular cavity. Clinical examination revealed cystic enlargement and tenderness of the ovaries. However, except for a few follicular hematomas that rupture into the abdominal cavity, there are generally no clinical symptoms.

Corpus luteum ovarian hematoma: similar to follicular hematoma. The hematoma generally involves the corpus luteum cavity, the lutein layer of granulosa cells and the lutein layer of theca cells. Rupture of the corpus luteum hematoma may cause intra-abdominal bleeding. In severe cases, the clinical manifestations may resemble ruptured fallopian tube pregnancy.

How to treat ovarian hematoma

Hematoma has no obvious clinical symptoms and usually disappears on its own without the need for treatment. However, when it ruptures or the ovary ruptures and bleeds, the treatment method will be determined based on the severity of the disease and the amount of internal bleeding. If the abdominal pain gradually eases or disappears and there are no signs of shock, it indicates that the condition is mild and the amount of internal bleeding is small. Conservative treatment can be performed, including bed rest, and the administration of hemostatic drugs and antibiotics to prevent pelvic infection. Small wounds can often close on their own and stop bleeding.

If the abdominal pain does not ease after the above conservative treatment and close observation, but gradually worsens, and there are obvious signs of anemia or even shock, then immediate laparotomy is required to stop bleeding and repair the ovary. If the repair is difficult and the ovary cannot be retained, oophorectomy is performed.
