Poor ovarian reserve refers to a decrease in the number of recruitable follicles remaining in the ovaries and a decline in oocyte quality, which can lead to reduced fertility or a tendency to premature menopause.

Age is the most important factor in determining ovarian reserve function. As age increases, the number and quality of follicles decrease significantly. Fertility begins to decline at the age of 30, and the prevalence of infertility increases significantly after the age of 35.

However, ovarian volume alone cannot be used as a predictor of ovarian reserve.

Generally, only 400 to 500 follicles can develop, mature and ovulate in a woman's lifetime. One dominant follicle is lost in each menstrual cycle. As women age, the number of follicles in their ovaries decreases until their ovaries atrophy after menopause.
