1.【Function and indications】

Amoxicillin is suitable for the following infections caused by sensitive bacteria (strains that do not produce β-lactamase):

1. Upper respiratory tract infections such as otitis media, sinusitis, pharyngitis, tonsillitis, etc. caused by hemolytic streptococci, pneumococci, staphylococci or Haemophilus influenzae.

2. Urinary and genital tract infections caused by Escherichia coli, Proteus mirabilis or Enterococcus faecalis.

3. Skin and soft tissue infections caused by hemolytic streptococci, staphylococci or Escherichia coli.

4. Lower respiratory tract infections such as acute bronchitis and pneumonia caused by hemolytic streptococci, pneumococci, staphylococci or Haemophilus influenzae.

5. Acute simple gonorrhea.

6. This product can also be used to treat typhoid fever, typhoid carriers and leptospirosis; amoxicillin can also be used in combination with clarithromycin and lansoprazole to eradicate Helicobacter pylori in the stomach and duodenum and reduce the recurrence rate of gastrointestinal ulcers.

2. [Usage and Dosage]

This product is taken orally.

1. For adults, take 0.5 g each time, every 6 to 8 hours. The daily dose should not exceed 4 g.

2. The daily dose for children is 20-40 mg/kg based on body weight, once every 8 hours; the daily dose for infants under 3 months old is 30 mg/kg based on body weight, once every 12 hours.

3. The dosage needs to be adjusted for patients with severe renal impairment. For patients with endogenous creatinine clearance of 10-30 ml/min, the dosage is 0.25-0.5 g every 12 hours; for patients with endogenous creatinine clearance of less than 10 ml/min, the dosage is 0.25-0.5 g every 24 hours.

5. Adverse Reactions

1. Gastrointestinal reactions such as nausea, vomiting, diarrhea and pseudomembranous enterocolitis.

4. Serum aminotransferase may be slightly elevated.

5. Superinfection caused by Candida albicans or drug-resistant bacteria.

It is contraindicated in patients with penicillin allergy or penicillin skin test positive.

1. A penicillin sodium skin test must be performed before use, and it is contraindicated for those with a positive reaction.

2. Patients with infectious mononucleosis are prone to rash when using this product and should avoid using it.

3. Patients who undergo longer treatment courses should have their liver and kidney function and blood routine checked.

4. Amoxicillin may cause false positive urine glucose tests using Benedict or Fehling reagents.

5. Use with caution in the following situations:

(1) Those with a history of allergic diseases such as asthma and hay fever.

(2) The dose may need to be adjusted in the elderly and patients with severe renal impairment.

9. [Use of medication in pregnant and lactating women]

Animal reproduction studies showed that amoxicillin at 10 times the human dose did not impair fertility or fetuses in rats and mice. However, there is a lack of adequate controlled studies in humans, and given that animal reproduction tests cannot fully predict human responses, pregnant women should use this product only when it is necessary. Since a small amount of amoxicillin can be secreted in breast milk, it may cause allergies in the baby after the breastfeeding mother takes it.

10. Drug interactions

1. Probenecid competitively reduces the renal tubular secretion of this product. The simultaneous use of the two can cause an increase in the blood concentration of amoxicillin and a prolonged half-life.

2. Chloramphenicol, macrolides, sulfonamides and tetracyclines interfere with the antibacterial effect of amoxicillin in vitro, but the clinical significance is unclear.

11. Pharmacological action

Amoxicillin is a penicillin antibiotic with good antibacterial activity against Streptococci such as Streptococcus pneumoniae and hemolytic Streptococcus, aerobic Gram-positive cocci such as non-penicillinase-producing Staphylococci and Enterococcus faecalis, non-β-lactamase-producing strains of aerobic Gram-negative bacteria such as Escherichia coli, Proteus mirabilis, Salmonella, Haemophilus influenzae, Neisseria gonorrhoeae, and Helicobacter pylori. Amoxicillin exerts its bactericidal effect by inhibiting the synthesis of bacterial cell walls, causing the bacteria to quickly become spherical and then dissolve and rupture.
