Dalteparin sodium is mainly suitable for common surgical diseases, patients who are bedridden for a long time, and patients with tumors. It can also be very helpful in preventing habitual abortion, so it still has effects on pregnant women. It is more suitable for alleviating some organ diseases that occur during pregnancy, including renal insufficiency. Dalteparin sodium currently also has adverse reactions. A small number of patients with abnormal liver function are prone to complications after taking dalteparin sodium, so pregnant women should consult a doctor while taking it. Indications 1. It can be used for deep vein thrombosis (DVT) in general surgery, total hip or knee replacement, long-term bedridden patients or patients with malignant tumors, and for the prevention of pulmonary embolism (PE), disseminated intravascular coagulation (DIC), etc. 2. Used for habitual abortion caused by positive lupus antibody. 3. It can significantly reduce the incidence of DVT in patients with acute ischemic stroke. 4. Prevent and treat thromboembolic diseases and prevent blood clot formation during hemodialysis. Contraindications 1. Patients with bleeding or bleeding tendency (especially caused by lack of certain coagulation factors). 2. Patients with systemic allergic reaction caused by dalteparin sodium. 3. Patients with thrombocytopenia and positive platelet aggregation reaction induced by dalteparin sodium in in vitro tests. 4. Patients with acute and subacute bacterial endocarditis. 5. Patients with progressive hemorrhagic organ damage (such as progressive gastric and duodenal ulcers). 6. Patients with cerebrovascular accident (except those with systemic disseminated intravascular coagulation), brain and spinal cord surgery. 7. Enoxaparin is contraindicated in patients with positive in vitro coagulation test, active digestive ulcer, allergy to dalteparin sodium and stroke (except those caused by systemic embolism). Precautions 1. (1) Patients with impaired liver and kidney function; (2) Patients with uncontrolled severe hypertension; (3) Patients with a history of peptic ulcer. 2. Effect of the drug on pregnancy: Dalteparin sodium should not be used in the first 3 months of pregnancy. 3. When dalteparin sodium is overdosed, protamine can be used as an antagonist. 4. Dalteparin sodium should not be injected intramuscularly. A platelet count should be performed before medication. 5. For patients with severe bleeding, 1 mg of protamine and 100 U of protamine sulfate can be slowly injected intravenously. Adverse Reactions Thrombocytopenia, abnormal liver function, and bleeding and ecchymosis at the injection site may be seen. Changes in transaminases and alkaline phosphatases are occasionally seen. There are reports that the use of dalteparin sodium during intrathecal epidural anesthesia and postoperative epidural catheter placement can lead to intraspinal hemorrhage, which can cause varying degrees of neurological damage, including long-term or permanent paralysis. |
