Imperforate hymen is a relatively common female problem. Imperforate hymen is also known as imperforate hymen. Hymen occlusion has a great impact on women's physical health because the vaginal opening is blocked by the hymen, which prevents menstruation from flowing out normally, causing abdominal pain and other problems. So what should we do if the hymen is imperforate?

1: Knowledge about hymen occlusion

1: Imperforate hymen, also known as imperforate hymen, is more common in clinical practice and is caused by the failure of the urogenital sinus epithelium to penetrate the vestibule. Experts from Quanzhou Mary Hospital pointed out that the internal reproductive organs of women with imperforate hymen mostly develop completely normally, and after entering puberty, the uterus still produces menstruation once a month.

Because the vaginal opening is blocked by the hymen, menstrual blood cannot flow out. As a result, after years of accumulation, the vagina and uterus are filled up, and can even flow back into the abdominal cavity through the fallopian tubes. The fallopian tube mucosa is squeezed and damaged by the accumulated blood, and the cilia of the epithelial cells disappear, losing the function of transporting sperm, eggs and fertilized eggs, making pregnancy impossible. The backflow of menstrual blood into the abdominal cavity can cause endometriosis and abdominal adhesions, causing severe abdominal pain.

2: Symptoms of hymen occlusion

1: There may be no symptoms before menarche. After menarche, menstrual blood cannot be discharged due to closure of the hymen. Initially, menstrual blood accumulates in the vagina. After multiple menstrual periods, the menstrual blood gradually accumulates, causing blood accumulation in the uterus, fallopian tubes, and even in the abdominal cavity.

However, the fimbria of the fallopian tube is often adhered and closed due to blood accumulation, so less menstrual blood enters the abdominal cavity. Girls with imperforate hymen usually have no clinical symptoms in the neonatal period. Occasionally, young girls are found to have a large amount of mucus accumulated in their vagina, causing the hymen to bulge outward.

3: Diagnosis and treatment of hymen imperforate

1: The vast majority of patients with imperforate hymen present clinically with gradually increasing cyclical lower abdominal pain after puberty, but without menstruation. In severe cases, there are symptoms such as constipation, anal distension, frequent urination or urinary retention. During the examination, the hymen was found to be bulging outward, the surface was purple-blue, and there was no vaginal opening. When the index finger is placed into the anus, a spherical mass can be felt in the vagina protruding toward the anterior wall of the rectum. During the rectal and abdominal examination, another smaller mass (the uterus with menstrual blood retention) can be felt in the lower abdomen above the vaginal mass, with obvious tenderness. If you press down on this lump with your hand, you can see the hymen bulge outward more obviously. Pelvic B-mode ultrasound examination can reveal fluid accumulation in the uterus and vagina.

Surgical treatment should be performed immediately after diagnosis. First, use a thick needle to puncture the central bulge of the hymen, and after drawing out the brown blood to confirm the diagnosis, the hymen is then cut open in a ";" shape to drain the accumulated blood. After most of the accumulated blood is discharged, a routine check is performed to see if the cervix is ​​normal, but further exploration of the uterine cavity is not advisable to avoid ascending infection. After all the accumulated blood is sucked out, the excess hymen is removed to make a circular incision, and then the mucosa at the edge of the incision is sutured with 3-0 absorbable sutures to maintain smooth drainage and prevent adhesion of the wound edge. A urinary catheter is placed for 1-2 days after the operation, a sterilized perineal pad is placed on the vulva, and the vulva is scrubbed 1-2 times a day until the accumulated blood is drained. Anti-infection drugs were given after surgery.

Four: Hymen occlusion as long as it can be timely

If discovered and treated in time, it will not affect fertility. If not treated in time, the accumulated blood will enlarge and deform the fallopian tubes, and blood clots will block the fallopian tubes, which will affect fertility after marriage.
