A colleague from the urology department told me about a special case of hematuria encountered in the emergency room, which is worth learning from. Once I was on night shift. There was always only one doctor on night shift in the urology department, who was responsible for both the ward and the emergency room. Because urology emergencies are relatively rare, not many people go to the emergency room for urology diseases, and those who are in critical condition are even rarer. Even if patients go to the emergency room, the most common condition is acute urinary retention, which often requires emergency catheterization, so the pressure is not very high when on duty. A very young woman came to the emergency room that day. She looked to be in her early 20s. She had a big bulge in her belly and was probably at least 7 or 8 months pregnant. The urologist was wondering if the patient came to see a doctor because the fetus was pressing on the bladder, causing frequent urination and urgency. The pregnant woman and her husband who accompanied her looked very anxious, and their words were a little incoherent. After the couple's relay explanations, the urologist finally found out: the patient was eight months pregnant and had no other abdominal pain or other discomfort. Today, she suddenly had blood in her urine, which scared the expectant parents. Will this cause harm to the fetus? Or could it be related to the fetus in the abdomen? Pregnant women with hematuria are different from ordinary patients and need to be cautious in many examinations and medication use. If one is not careful and the fetus is affected, it will be a serious matter and the urologist will be under great pressure. The causes of bleeding in urine are mainly analyzed by urologists as follows: 1. Inflammation of the urinary and reproductive system. Such as: chronic pyelonephritis, acute cystitis. 2. Tumor tuberculosis. Such as: bladder tumor, kidney tumor, bladder tuberculosis, kidney tuberculosis. 3. Kidney disease. Acute glomerulonephritis, chronic glomerulonephritis, nephrotic syndrome. 4. Urinary tract infection. Bleeding in urine is often accompanied by urgency, pain, and frequency of urination. 5. Urinary tract stones: Not only will there be bleeding in the urine, but there will also be severe pain in the waist and abdomen. The most common causes of blood in urine are urinary tract stones and urinary tract infections. However, through physical examination of the pregnant woman, no percussion pain was found in the kidney area, and the individual's symptoms were not obvious enough. The cause of the patient's hematuria cannot be determined for the time being, and further objective examination and differential diagnosis are needed. Abdominal X-ray, CT, B-ultrasound, etc. have high differential diagnostic significance, but may have an impact on pregnant women, so the simplest method is used first, which is to check urine routine. After 15 minutes, the test results were shocking. The urine routine test was completely normal, even better than before, with no signs of bleeding or infection. What could be the problem? The urologist thought that there might be other problems, and asked the couple carefully what they had eaten that day. Both husband and wife were a little confused. After becoming pregnant, they basically didn’t dare to eat out. They only had ordinary home-cooked meals! The fruits change every day... Today I ate red dragon fruit! I was 8 months pregnant and suddenly had blood in my urine, all because I ate this fruit! I see! Urologists are at a loss whether to laugh or cry. Foods like red dragon fruit, mulberries, red amaranth, watermelon, purple sweet potato, tomato, and even some dyed preserved fruits contain some pigments, which are not needed by the human body and will be excreted through urine after absorption. But not everyone will have red urine, it depends on personal constitution. Some people have stronger abilities to absorb and metabolize these pigments, so their urine will naturally not change color. Finally, I would like to remind everyone that we must protect our urinary system in our daily lives. Drink plenty of water and eat more fresh fruits and vegetables every day; avoid overfilling the bladder and urinate in time; take good protective health care against chemical industrial products; refuse to overeat, pay attention to the combination of work and rest, and prevent urinary system bleeding. |
