Uterine cyst, also known as uterine leiomyoma, is the most common benign tumor of the female reproductive system. Also known as fibroids and uterine fibroids. Since uterine fibroids are mainly formed by the proliferation of uterine smooth muscle cells, with a small amount of fibrous connective tissue existing as a supporting tissue, it is more accurate to call them uterine leiomyomas. Most cases are asymptomatic, while a few may present with vaginal bleeding, abdominal masses, and compression symptoms. It may cause pain if the pedicle is twisted or other conditions occur. Multiple uterine fibroids are common. The exact cause of this disease is unknown, but it may be related to excessive estrogen levels in the body and long-term estrogen stimulation. Modern Western medicine uses sex hormones or surgical treatment, and there is no other ideal treatment.

Uterine cyst is a rare disease that is easily overlooked clinically. It can be divided into congenital and acquired uterine cysts. The former originates from the mesonephric duct and paramesonephric duct, and mostly occurs in the posterior wall or fundus of the uterus; the latter are mostly secondary to benign diseases or develop from the interstitial cells of the serosa, including cystic degeneration of uterine fibroids, cystic adenomyoma, cervical retention cysts and uterine serosa cysts.

Clinical manifestations of uterine cysts:

1. The age of onset is between 25 and 52 years old, with an average age of 36 years old. .

2. Symptoms Congenital uterine cysts generally have no obvious subjective symptoms, and sometimes a lump can be felt in the lower abdomen.

3. Signs of uterine cysts include bulging of the uterine fundus or posterior wall, with a local cystic sensation. It can be divided into three types: subserosal, intramural and submucosal. If it is a pedunculated submucosal cyst, it can sometimes protrude outside the cervical os.

Pathological examination of uterine cysts shows that the cyst wall originating from the mesonephric duct is mostly composed of columnar or stratified squamous epithelium. The cyst wall originating from the paramesonephric duct is mostly composed of ciliated columnar epithelium.

Differential diagnosis of uterine cysts:

(I) Ovarian cyst

Ovarian cysts generally have no clinical symptoms and are only discovered during regular gynecological examinations. Gynecological examination may reveal cystic tumors on one or both sides of the uterus, which are separable from the uterus and do not move with uterine activity. Ultrasound examination of the cyst in relation to the uterus can assist in diagnosis.

(ii) Cervical polyps

If there is a protrusion from the cervical os, it should be distinguished from a pedunculated submucosal cyst of the uterus. Cervical polyps are mostly single, but there are also multiple ones. They have pedicles, a smooth surface and are red in color. Because polyps contain abundant blood vessels, they are prone to rupture and bleed, which can cause irregular vaginal bleeding. Polyp removal and pathological examination can confirm the diagnosis.
