Benign changes may have the following situations:

Hyaline degeneration, due to the rapid growth of the tumor, causes relative blood deficiency, resulting in edema of some tissues, disappearance of the vortex-like structure, and replacement by a uniform transparent substance. It is easily confused with sarcoma degeneration during gross examination. No cell structure can be seen under the light microscope, and the lesion is a structureless, uniform eosin-colored area.

Necrosis is caused by tumor pedicle torsion or severe infection. The central part of the tumor is far away from the blood supply and is most susceptible to necrosis. The tissue is grayish yellow and brittle, and small cavities may form.

Infection is more common in patients whose submucosal fibroids protrude into the vagina, where blood supply is blocked, necrosis occurs, and then infection occurs. A small number of patients also have infected lesions in the pelvis, which often affect uterine fibroids.

Fatty degeneration often occurs in the late stage of hyaline degeneration or after necrosis, and may also be the result of mesenchymal metaplasia of the tumor to form adipose tissue. The texture is soft and can easily be diagnosed as a sarcoma. Under light microscopy, vacuoles were seen in the muscle cells and fat staining was positive.

Cystic change is caused by the further development of hyaline degeneration. On the basis of hyaline degeneration, insufficient blood supply causes liquefaction of tissue in the degenerated area, forming a cystic cavity containing jelly-like or transparent fluid. The entire tumor is soft like a cyst.

Red degeneration is a special type of tumor necrosis, which is more common in single larger intramural myomas and often occurs during pregnancy or the postpartum period. It may be related to local tissue ischemia, infarction, congestion, and thrombosis, which lead to local tissue bleeding and hemolysis, causing blood to infiltrate the tumor. The myoma appears red to the naked eye, resembling raw beef, and completely loses its original spiral structure.

Calcification, a rare occurrence, is seen primarily in cases of chronic myoma insufficiency, such as in postmenopausal women or in myomas with thin pedicles. If the calcium blue deposits are scattered and sparse, they cannot be identified generally, but will feel sandy when the tumor is dissected; if the lesion is extensive, it will form a hard stone. The calcified area under microscopic examination is dark blue, forming layered deposits of varying sizes and shapes. X-ray films can be developed.

Malignant transformation

Sarcoma degeneration: About 1% of uterine fibroids transform into sarcomas, which is more common in older women with larger and faster-growing fibroids, especially in patients with rapidly growing fibroids after menopause or reappearing fibroids after menopause. The mechanism is unknown. The tissue in the sarcoma lesion area is gray-yellow and soft like raw fish meat.

Judging from the degeneration of uterine fibroids, although uterine fibroids are benign tumors, if not treated in time, they will delay the disease and even endanger life. From this we can see that uterine fibroids will not heal on their own. Therefore, women with uterine fibroids should seek early treatment to avoid missing the best treatment time. Therefore, you should be especially vigilant if fibroids do not shrink after menopause but continue to grow. Some patients whose fibroids turn into sarcomas do not have any clinical symptoms. Subserosal and intramural fibroids may metastasize through the peritoneum and cause pain and adhesions. In the late stage of sarcoma, patients often suffer from anemia, extreme weight loss, and cachexia.
