Vaginal cysts are the most common benign vaginal tumors. There are no glands in the normal vagina, but isolated aberrant crypts can occasionally be found, which form retention cysts containing fluid. They are not neoplastic or proliferative tumors. Generally, the cyst epithelium originates from the Müllerian duct, mesonephric duct and urogenital sinus during the embryonic period. Ovarian coronal duct cysts originate from the events of the medullary duct.

Treatment

Surgical resection is the main treatment. If the cyst is not too high, the operation is usually not difficult, but care must be taken during the removal process to avoid injuring the urethra or bladder. If the tumor is large and located deep in the fornix and extends into the broad ligament, it is impossible to completely remove it vaginally, and it is very difficult even if surgery is performed abdominally at the same time. Some authors believe that after scraping the remaining cyst wall with a curette, the edges of the stump are sutured together with the corresponding edges of the vaginal mucosal incision to form a stoma, and then the vagina is filled with gauze to compress the remaining cyst cavity. It is possible to make the remaining cyst wall completely adhere and close. Even if it cannot be adhered and closed, it will not inflate again.

Clinical manifestations

The cysts may be segmented or multiple, and vary in size, generally 2 to 3 cm in diameter, with a smooth, fixed appearance and a cystic feel to the touch. This cyst is often small and has no clinical significance, but occasionally it can grow very large and cause difficulty or pain in sexual intercourse, or even hinder childbirth. It sometimes compresses the bladder trigone and causes increased urination frequency. In rare cases, its long, cord-like pedicle can cause bowel distortion and obstruction.

The contents of the cyst are mostly watery, serous or milky white liquid, and some are dark brown. Its color and viscosity vary depending on the presence or absence of intracystic bleeding and the amount of bleeding.
