As labor progresses, uterine contractions continue to strengthen, the upper uterine muscles over-contract and retract, becoming thicker and thicker, while the lower muscles passively expand and stretch, becoming thinner and thinner, forming an annular depression between the upper and lower segments. This depression can gradually rise as high as the navel or even above the navel as the uterus contracts, which is called a pathological retraction ring. It is a precursor to uterine rupture. When uterine rupture is threatened, the lower segment of the uterus is swollen and tender, and a pathological contraction ring can be seen. The mother will show symptoms of irritability, accelerated breathing and heart rate, and unbearable pain in the lower abdomen; the bladder will be compressed and congested, causing difficulty in urination and hematuria. If not treated quickly, the uterus will rupture at or below the pathological contraction ring. Due to frequent and strong uterine contractions, the blood supply to the fetus is blocked and the fetal heart rate changes or becomes unclear.

As labor progresses, the fetal shoulder is squeezed into the pelvic entrance, and the fetal neck further flexes laterally, causing the head to bend toward the ventral side of the fetus and become incarcerated in the iliac fossa on one side, while the fetal buttocks are incarcerated in the opposite iliac fossa or the folded upper part of the uterine cavity. The upper limb on the side with shoulder presentation prolapses into the vagina, forming a neglected (incarcerated) shoulder presentation, which directly hinders the progress of labor and causes labor stagnation. If the uterine contractions are too strong at this time, a pathological contraction ring may be formed, and there is a risk of uterine rupture.
