On the 3rd to 4th day of a normal menstrual period, the secretory endometrium has completely shed and is replaced by regenerated proliferative endometrium. However, when the endometrium is shed irregularly, the endometrium showing secretory reaction can still be seen on the 5th-6th day of the menstrual period.

Because the patient's menstrual period is long, the endometrium loses water, the interstitium becomes dense, the glands shrink, and the glandular cavity becomes plum blossom or star-shaped. The endometrium showed a mixed type, that is, the residual secretory endometrium coexisted with hemorrhagic necrotic tissue and newly grown endometrium. In addition to typical clinical manifestations, irregular endometrial shedding causes the basal body temperature to be biphasic but to decrease slowly.

The diagnostic curettage for irregular endometrial shedding is performed on the 5th to 6th day of the menstrual period. The endometrial biopsy can still show the endometrium with secretory reaction, which coexists with the bleeding and proliferative endometrium. The corpus luteum generally shrinks after 14 days, and the endometrium falls off due to lack of support from estrogen and progesterone, causing menstruation.

After diagnostic curettage and hysteroscopy to exclude hemostasis and malignancy, sequential treatment with hormonal drugs can be used.
