In recent years, with the increasing incidence of malignant tumors, it has gradually become the number one killer of human health, and endocrine metabolic diseases such as diabetes and thyroid disease are also severely affected areas. There are more than 100 million diabetes patients alone.

"There is a close connection between malignant tumors and various endocrine and metabolic diseases such as diabetes, thyroid disease, and abnormal bone metabolism, but cross-disciplinary cooperation between the two disciplines is rare in China," said Wu Qinan, head of the Department of Endocrinology and Nephrology at the Affiliated Cancer Hospital of Chongqing University.

First of all, many cancer patients suffer from endocrine tumors, such as thyroid cancer, pituitary tumors, adrenal tumors, etc. The development and metastasis of some tumors are also related to endocrine hormones, such as breast cancer and prostate cancer related to sex hormones.

Secondly, with the improvement of tumor diagnosis and treatment and the significant extension of survival time, many patients with non-endocrine tumors may develop a variety of endocrine and metabolic complications.

Finally/with the advancement and updating of treatment methods, anti-tumor drugs lead to many adverse endocrine and metabolic reactions such as thyroid dysfunction, blood sugar disorder, abnormal bone metabolism, adrenal dysfunction, pituitary dysfunction, sexual dysfunction and electrolyte disorder.

"Taking tumors and diabetes as an example, a large amount of research and data has confirmed that diabetic patients have a higher risk of developing malignant tumors." Wu Qinnan said that the more common malignant tumors in diabetic patients include liver cancer, kidney cancer, pancreatic cancer, rectal cancer, bladder cancer and lung cancer.

Research surveys have shown that male diabetic patients are 27% more likely to develop cancer than men without diabetes, and respondents with fasting blood sugar levels higher than 7.8mmol/L are 25% more likely to die from cancer than those with fasting blood sugar levels lower than 5.6mmol/L.

Malignant tumor treatment drugs, radiotherapy, and immune checkpoint inhibitors may also increase the incidence of diabetes. At the same time, patients with tumor-related diabetes have a lower blood sugar control rate and a worse prognosis than those with ordinary diabetes.

"Therefore, it is very necessary to open a tumor endocrinology clinic and establish a tumor endocrine MDT in the future." Wu Qinnan said that this has practical significance for standardizing the risk assessment and diagnosis and treatment of tumor endocrine patients, solving the clinical problems of difficult and serious tumor endocrine patients, and treating patients with malignant tumors more accurately.

Wu Qinan said that two types of cancer patients need:

■ Patients with endocrine and metabolic diseases caused by tumor treatment, such as elevated blood sugar and electrolyte imbalance;

■ Tumor patients have combined endocrine and metabolic diseases such as diabetes, hyperthyroidism, etc.

Wu Qinan said that as an emerging discipline, future development faces many challenges: innovation and change of concepts, urgent transformation of diagnosis and treatment models, strengthening of training of tumor endocrinology specialists, urgent strengthening of related research in tumor endocrinology, and good collaboration among multiple disciplines in the discipline of tumor endocrinology.

At the same time, it is also full of opportunities and hope. The Department of Endocrinology and Nephrology of Chongqing Cancer Hospital/Affiliated Cancer Hospital of Chongqing University integrates medical treatment, scientific research and teaching, and is committed to the prevention, diagnosis, treatment, rehabilitation and education of endocrine metabolic diseases and kidney diseases.

There are more than 20 medical staff members, including 2 deputy chief physicians, 4 attending physicians, 3 with doctoral degrees, and the rest with master's degrees in the medical series; more than 80% of the nursing series have bachelor's degrees.

The department consists of 1 ward, 1 blood purification center and 1 instrument inspection room, and is currently establishing a national standardized metabolic disease management center.

The ward has 53 beds and is equipped with a real-time dynamic insulin pump, a dynamic blood glucose monitoring system, a dual-energy X-ray bone densitometer, a blood gas analyzer, a body fat analyzer, an ophthalmoscope, a peripheral vascular diagnostic system, a sensory nerve quantitative tester, a transcutaneous oxygen partial pressure, a diabetic foot diagnostic toolbox, a neurovascular therapeutic device, an osteoporosis therapeutic device, a hemodialysis machine, an ECG monitor, etc.

The department has a patient education team, blood sugar management team, diabetic wound management team, endocrine and metabolic critical care management team and kidney disease team.

The diagnosis and treatment projects offered include: diabetes and acute and chronic complications (diabetic ketoacidosis/hyperosmolar state, diabetic foot, diabetic nephropathy, peripheral neuropathy/vascular disease and retinopathy, etc.), thyroid diseases (including hyperthyroidism, hypothyroidism, thyroiditis, thyroid nodules, etc.), osteoporosis, gout, dyslipidemia, obesity, electrolyte disorders, pituitary diseases, adrenal diseases, sexual development diseases, acute and chronic nephritis, acute and chronic renal failure, urinary tract infections, etc. In particular, the diagnosis, treatment and management of tumor-related endocrine kidney diseases are formed in combination with the oncology specialty, which is unique in the country and Chongqing.
