Many hypertensive patients told Huazi that they insisted on taking medicine according to the doctor's advice, but their blood pressure was not well controlled. What was going on? In the process of answering questions, Huazi found that some people's hypertension types did not belong to "classic hypertension", but each had its own characteristics. Only by "fine-tuning" according to each person's characteristics can they help them better control their blood pressure.

1. Classic hypertension Classic hypertension means that both high and low blood pressure are elevated, and the daily blood pressure fluctuation pattern conforms to the normal "two peaks and one valley", that is, there are two peaks in blood pressure in the morning and afternoon, and the blood pressure value drops to the lowest at night. If the daily blood pressure fluctuation pattern is drawn into a curve, it looks like a "spoon", so it is also called "spoon-shaped blood pressure".

Most patients with hypertension have typical hypertension. The treatment is relatively simple. Choose long-acting antihypertensive drugs. Normally, take the medicine on an empty stomach after getting up in the morning. The blood drug concentration is higher during the day, which can well control the blood pressure value for the whole day, while preventing the blood pressure from dropping too low at night.

2. Nighttime hypertension and morning hypertension Some patients' blood pressure fluctuations are not so "regular", showing low blood pressure during the day, high blood pressure at night, or high blood pressure in the early morning. This is the opposite of the normal blood pressure fluctuation pattern, also known as "reverse spoon-shaped blood pressure". For such blood pressure, normal medication methods cannot be used, otherwise the daytime low blood pressure will be aggravated, and the nighttime and early morning high blood pressure will not be adequately controlled, which is prone to danger.

Therefore, for such patients, they need to take long-acting antihypertensive drugs at night or before going to bed to better control blood pressure at night and in the early morning and reduce the risk.

3. Blood pressure with excessive pulse pressure Blood pressure is divided into high pressure (systolic pressure) and low pressure (diastolic pressure). The difference between the two blood pressures is called pulse pressure, or pulse pressure difference. When the difference is too large (more than 60mmHg), it means that large artery sclerosis is serious. The blood pressure of elderly hypertensive patients often shows this characteristic of high pressure increase and low pressure unchanged or decreased, which is called isolated systolic hypertension.

This type of blood pressure with only elevated systolic pressure is usually sensitive to sodium salt and is a type of volumetric hypertension. Commonly used long-acting antihypertensive drugs such as diazepam, pril, and sartan are not effective. It is necessary to add diuretics to the routine use of long-acting antihypertensive drugs to promote water and sodium excretion in order to better control blood pressure.

4. High diastolic blood pressure The common blood pressure type among young and middle-aged people is that the diastolic blood pressure is elevated, the diastolic blood pressure is normal or slightly elevated, and the pulse pressure is too low (about 20 mmHg). This is because the elasticity of the large arteries of young and middle-aged people is still acceptable, but due to mental stress or obesity, the sympathetic nerves are excited, causing the peripheral arterioles to contract, usually accompanied by an increased heart rate (greater than 80).

For those with a heart rate greater than 80, the use of loreta drugs is more effective, which can lower blood pressure and slow down the heart rate. For those with a heart rate less than 80, sartan drugs can be considered.

5. Postural hypotension and postprandial hypotension Elderly hypertensive patients will experience postural hypotension when they switch from lying or sitting to standing due to their insufficient blood pressure regulation function. If you are thin, you should drink more water to replenish blood volume. If you have varicose veins in your lower limbs, you should wear elastic stockings to help venous blood return. You should change your body position slowly.

Elderly patients with hypertension may also experience postprandial hypotension. This is manifested by a drop in high blood pressure of more than 20 mmHg within 2 hours after a meal, accompanied by symptoms of insufficient blood supply such as dizziness, drowsiness, and falls. This often occurs after taking diuretics and vasodilators. You can choose to take antihypertensive drugs 1 to 2 hours after a meal to reduce postprandial hypotension. After a meal, sit and rest for half an hour before getting up.

In summary, patients with hypertension need to know the characteristics of their blood pressure in order to choose appropriate antihypertensive drugs and determine the specific time to take the drugs.
