The position of the uterus is forward-flexed, with the fundus of the uterus located two fingers' width below the navel. In other words, when a woman is standing, the uterus is basically in a straight line and slightly bent forward, the fundus of the uterus is on the bladder, the cervix is ​​posteriorly toward the bottom of the sacrum, and the external os of the cervix is ​​approximately at the level of the ischial spines. What maintains this position? It turns out that the uterus has four pairs of cruciate ligaments, as well as the muscles and muscle fascia of the pelvic floor, which keep the uterus in its normal position. The four pairs of cruciate ligaments are:

1. Round ligament, which originates from the front of the two corners of the uterus and develops from the front of the labia majora. Its function is to keep the fundus of the uterus in an extended position.

2. The broad cruciate ligament is a pair of wing-shaped retroperitoneal folds that start from both sides of the uterus and reach the pelvic wall.

3. The main cruciate ligament is below the broad cruciate ligament and runs between the two sides of the cervix and the pelvic wall. It is a pair of strong smooth muscle and connective tissue fiber bundles, also known as the transverse cruciate ligament of the cervix. It is an important tissue that fixes the position of the uterus.

4. The cruciate ligament of the uterine sacrum runs from the upper side behind the cervix, around the duodenum on both sides, and reaches the front of the second and third sacrums. The cruciate ligament contains smooth muscle and connective tissue. It is short, thick and strong. It can pull the cervix backward and upward, indirectly maintaining the uterus in a protruding position. Therefore, the maintenance of the normal position of the uterus depends on the support of the above four pairs of cruciate ligaments and the muscles and muscle fascia of the pelvic floor.
