Metrorrhagia is mostly caused by excessive yang in the body, or exposure to heat evil, or liver depression transforming into fire, yin deficiency and essence deficiency, internal disturbance of virtual heat, and excessive heat in the blood; spleen deficiency, overeating and fatigue, which damage the spleen and make it unable to control the body; emotional discomfort, or exposure to cold and dampness, blood stasis, and blood not flowing through the meridians; early marriage and early childbearing, multiple births, labor, long-term illness that damages the kidneys, and poor storage, etc.

Common syndromes include:

①Metrorrhagia due to Yin deficiency and blood heat. Symptoms include irregular menstruation, continuous or sudden heavy menstruation, dark red blood, hot flashes, dry mouth and throat, hot hands and feet, or insomnia, irritability, red tongue with yellow coating, and a weak and rapid pulse. The treatment should be to nourish yin and clear away heat, cool blood and consolidate the meridians. The prescription used can be Qingre Gujing Decoction, etc.

②Metrorrhagia due to liver depression and blood heat. Symptoms include irregular menstruation, intermittent bleeding, dark red and thick menstruation, irritability, breast swelling, red tongue with yellow coating, and a stringy and rapid pulse. The treatment should be to clear away heat, relieve depression and stop bleeding, and the prescription used can be Danzhi Xiaoyao Pills, etc.

③Metrorrhagia due to spleen deficiency and qi collapse. Symptoms include sudden hemorrhage or continuous dripping, light and thin blood, sallow complexion, shortness of breath, poor appetite, chest tightness, palpitations, or loose stools, pale tongue, and weak pulse. The treatment should be to replenish qi and retain blood, and the prescriptions used include Guben Zhibeng Decoction, Guipi Decoction, etc.

④Blood stasis type metrorrhagia. Symptoms include continuous menstrual bleeding or sudden heavy bleeding, dark purple blood with clots, lower abdominal pain that is resistant to pressure, pain relief after the clots are discharged, dark red tongue with petechiae, and deep and sluggish pulse. The treatment should be to promote blood circulation, remove blood stasis and stop bleeding. The prescription used can be Taohong Siwu Decoction, etc.

⑤Metrorrhagia due to kidney deficiency. Symptoms include small amount of bleeding, continuous bleeding, or sudden large amount of bleeding, light red or dark purple in color, soreness and weakness in the waist and spine, empty lower abdomen, or insomnia, night sweats, irritability, or chills, loose stools, red or pale tongue, and thin and rapid or deep and weak pulse.
