Zoledronic acid is used in daily life to help people treat bone-related diseases and to help people prevent and treat myeloma. Therefore, zoledronic acid is widely used in medicine. The mechanism of action of zoledronic acid has not been fully understood so far, but it is guaranteed that it can inhibit bone cell activity and thus help people relieve bone pain. Zoledronic acid inhibits bone resorption but has no adverse effects on bone formation, mineralization, and mechanical properties. Zoledronic acid injection is mainly used for hypercalcemia caused by malignant tumors. It can reduce the risk of multiple myeloma and has anti-tumor cell effects. It can reduce and delay the occurrence of bone complications. It is the only bisphosphonate in the world that has been proven to significantly prolong the survival of multiple myeloma patients. The pharmacological action of zoledronic acid is mainly to inhibit bone resorption. Its mechanism is not fully understood and may be related to multiple effects.

Zoledronic acid can inhibit osteoclast activity in vitro, induce osteoclast apoptosis, and block osteoclast absorption of mineralized bone and cartilage by binding to bones. Zoledronic acid is often used clinically to relieve bone pain caused by osteolytic bone metastases of malignant tumors.

In addition, zoledronic acid can inhibit the enhanced osteoclast activity and bone calcium release caused by various stimulatory factors released by tumors. Therefore, after zoledronic acid attaches to the surface of the bone, it is equivalent to a layer of "bone coat". Externally, it can inhibit the erosion of osteoclast factors released by the tumor, and internally it can change the osteoblast-osteoclast homeostasis toward osteogenesis, relatively promoting bone formation and reducing bone calcium release.
