Trichomonas vaginitis is the most common type of vaginitis among female gynecological diseases. It is mainly caused by the presence of Trichomonas in the vagina. Trichomonas have strong survival energy and can survive for two to three days regardless of low or high temperatures. Trichomonas have a strong ability to adapt to the environment and are easily spread. It can be transmitted not only sexually but also in many other ways. Direct infection (30%):

Transmitted through sexual intercourse, indirectly transmitted: through public baths, bathtubs, baths, swimming pools, toilets, clothing, equipment and dressings, etc. Trichomonas vaginitis is acquired through external infection. For an infectious disease to spread, it must have three conditions: source of infection, route of transmission and susceptible person.

Anyone can be infected with Trichomonas, and those with changes in vaginal acidity or low immunity are more susceptible to infection. Very few patients can recover on their own after being infected with Trichomonas, and even if they are cured, they can be infected again next time. The sources of infection of trichomoniasis are those carriers and contaminated objects.

Sexual transmission (30%):

Vaginal trichomoniasis is mainly transmitted through sexual intercourse, so it is classified as a sexually transmitted disease. In my country, the way it is transmitted is different. Due to my country's large population and relatively backward public health facilities compared to developed countries, transmission in public places has also become an important route of transmission. For example, seats in public baths or toilets in public toilets are contaminated with the secretions of carriers. If later people sit directly on the seats or toilets, they may be infected. The basins in public baths, swimming pools with high density and poor disinfection in summer, wearing other people's underwear, renting swimsuits, etc. may all cause the spread of Trichomonas.

In addition, family members sharing bathtubs and medical cross-infection are also causes of indirect transmission of Trichomonas.

Parasite transmission (30%):

Gynecological experts suggest: The pathogen of this disease is Trichomonas vaginalis, which is a parasite but invisible to the naked eye. It is pear-shaped, 10-30μm long, with 4 flagella on its head that are as long as the body of the worm. It can be clearly seen under a microscope. Trichomonas has a strong adaptability to different environments and can grow and reproduce at 25℃-42℃, survive at low temperatures of 3℃-5℃ for 21 days, and can still survive at 46℃ for 20-60 minutes. It can survive for several hours in semi-dry conditions after leaving the human body. Trichomonas not only parasitizes in the oxygen-deficient vagina, but can also invade the urethra and paraurethral glands, and even ascend to the ureter and renal pelvis. The most suitable pH value for the growth of Trichomonas is 5.5-6. If the pH is below 5 or above 7.5, the growth of Trichomonas will be inhibited.

Most cases are asymptomatic. Women may feel uncomfortable for a week or several months, and then improve significantly due to menstruation or pregnancy. The vaginal mucosa is inflamed, bright red, covered with patchy pseudomembranes, often accompanied by foamy secretions, and they feel itching to varying degrees, and a few have a burning sensation. Leucorrhea increases and turns yellow-green. Frequent urination, urgency, pain, hematuria, abdominal pain, diarrhea, mucous stools, alveolar pus discharge, and tooth decay may occasionally occur. It often causes urethritis and can lead to cystitis and Bartholin's glanditis.

The diagnosis can be made immediately by suspending the vaginal secretions taken from the posterior fornix in saline and examining them under an ordinary microscope without staining. The rapid extension movement of the flagella and the sprinting activity of the ovoid protozoa were easily observed. Culture is more sensitive than direct microscopy. Trichomonas vaginitis is also often diagnosed using a Papanicolaou-stained smear. Tests should be done to rule out gonorrhea, chlamydia, and other sexually transmitted diseases.
