(1) Immunosuppressants ① Methotrexate (MTX) works by competitively inhibiting dihydrofolate reductase and most of them are effective within 1 to 2 weeks. Side effects include anorexia, nausea, discomfort, oral mucosal ulcers, leukocytopenia and thrombocytopenia, and liver function damage. When using this drug, you should pay attention to checking blood routine before taking the drug and check it regularly. ②Hydroxyurea is suitable for extensive, stubborn psoriasis, pustular psoriasis and erythrodermic psoriasis. The main side effects are bone marrow suppression, occasional gastrointestinal reactions, central nervous system symptoms, hair loss, fatigue, loss of libido, dizziness, and teratogenesis in some cases. Liver and kidney functions are less affected. ③Cyclosporin is used to treat pustular psoriasis, arthritis psoriasis, and generalized plaque psoriasis that is resistant to conventional treatment. The treatment will take effect within 3 to 7 days, and most cases can be clinically cured within 3 to 4 weeks. The main adverse reactions include gastrointestinal symptoms, urinary tract irritation symptoms, increased blood pressure, fatigue, muscle tremors, etc. ④ The mechanism of action of tacrolimus is the same as that of cyclosporine, which mainly inhibits the activity of calcineurin and the activation of T lymphocytes. It is effective in the systemic treatment of psoriasis. ⑤ Total glycosides of Tripterygium wilfordii are effective in treating psoriasis and are also effective for pustular psoriasis. Gastrointestinal reactions and menstrual disorders are the most common side effects. Taking it after meals can reduce gastrointestinal reactions. It may occasionally cause pigmentation, lip erosion, thinning nails, hair loss, decreased white blood cell count and mild liver damage.

(2) Retinoic acid drugs are intermediate products or similar derivatives of vitamin A metabolism in the body. They mainly affect epithelial metabolism, promote epidermal cell proliferation and differentiation, dissolve keratin, and maintain the normal structure of the skin. Used to treat various types of psoriasis, especially erythrodermic psoriasis, localized or generalized pustular psoriasis. The main side effect of this class of drugs is teratogenicity. During medication, the lips, eyes, and nasal mucosa may become dry, and the skin may become diffusely desquamated, cracked, and itchy.

(3) Antibiotics: Penicillin and erythromycin can be used for patients with progressive psoriasis vulgaris and guttate psoriasis accompanied by tonsillitis and infection.

(4) Corticosteroids Corticosteroids can often improve the skin lesions of psoriasis, but cannot prevent its recurrence. It is not suitable for use in patients with psoriasis vulgaris. It is only used for patients with erythrodermic, arthritic or generalized pustular psoriasis who have not responded to other drugs, or for short-term use for patients with concurrent symptoms of systemic poisoning. This product is often used clinically in combination with other therapies to consolidate or maintain symptom relief. When used in combination with immunosuppressants and retinoic acid, the dosage can be reduced. Prednisone is commonly used, and oral corticosteroids are not recommended because the effective dose is large enough to cause serious side effects. "Rebound" may occur after the dose is reduced or the drug is stopped.

5) Thiamphenicol Thiamphenicol is used to treat pustular psoriasis. It has a rapid effect, high efficacy and few side effects. It is one of the first choice drugs for the treatment of pustular psoriasis. It can be taken orally alone or combined with corticosteroids. The main side effects include bone marrow suppression and gastrointestinal symptoms such as loss of appetite, nausea, vomiting, abdominal pain, and diarrhea. Use with caution in pregnant women.
