(1) The shape of the bladder: When depressed, the bladder is similar to a cone, divided into three parts: tip, base and body. The bladder is small and pointed, facing forward and upward; the base of the bladder is triangular, with the housing facing downward and backward, and the urethra enters the bladder wall at the upper corners of the base of the bladder; the part between the tip and base of the bladder is called the bladder body. There is no obvious boundary between the bladder parts, and it becomes oval when the bladder is full. (B) Bladder location: The adult bladder is located in the pelvis, just behind the ischial tuberosity. When the bladder is painful, the bladder does not extend beyond the upper edge of the ischial tuberosity. When the profit is in the balance, there will be different levels of increase. When it is extremely full, it can be higher than the upper edge of the ischial tuberosity. The anterior and inferior wall of the bladder is just behind the ischial tuberosity. In men, the bottom of the bladder directly touches the urethra, the tail end of the ejaculatory duct and the seminal vesicle, and then touches the duodenum backwards; in women, the bottom of the bladder touches the uterus and the vagina. Directly below the bladder, men are adjacent to the male prostate; women are adjacent to the urogenital diaphragm. The parietal peritoneum of the anterior abdominal wall is folded back behind the ischial tuberosity, covering the top and sides of the bladder. There is no retroperitoneum in front of the bladder. When the bladder height is filled and rises, the retroperitoneum under the anterior abdominal wall moves upward with the rise of the bladder, and the anterior bladder wall is directly attached to the anterior abdominal wall. Therefore, puncture or cystostomy can be performed above the ischial tuberosity through the anterior abdominal wall without going directly to the bladder through the peritoneal cavity. (III) Structure of bladder wall The bladder wall is composed of mucosa, submucosa, muscularis and adventitia. When the bladder is contracted, the mucosa gathers into many wrinkles, and when it is filled, the wrinkles disappear. At the bottom of the bladder, between the left and right urethral openings and the internal opening of the urethra, there is a triangular area called the bladder triangle. This area lacks submucosal tissue, and the mucosa is smooth and has no wrinkles. It is a common location for tumors, tuberculosis and cystitis. |
