Follicular cysts are formed due to degeneration of the follicular epithelium, hyperplasia and thickening of the connective tissue of the follicular wall, death of the oocyte, and unabsorbed or increased follicular fluid.

The blood vessels in the theca layer of the ovarian gland suddenly rupture for some reason, and blood enters the follicular cavity to form follicular hematoma. The hematoma is gradually absorbed, the blood cell components melt and disappear, and the fluid increases and accumulates to form a cyst. Inflammation causes the mature follicle to fail to rupture and ovulate, and the secretory fluid in the follicle is retained to form a cyst.

The follicle-stimulating hormone in the pituitary gonadotropin is over-secreted or the follicle-stimulating hormone and luteinizing hormone are out of balance, resulting in the mature follicle not ovulating but continuing to grow or the atretic follicle incompletely degenerating and the granulosa cells still secreting fluid to form cysts.
