Patients usually have a history of dysmenorrhea, which often starts one or two days before menstruation, lasts throughout the menstrual period, and can still occur a few days after the menstrual period ends. Some women also have problems such as excessive menstrual bleeding, painful intercourse and infertility. However, about half of the patients are asymptomatic, because after hysterectomy for other diseases, pathological examination accidentally discovered adenomyosis. Internal examination may reveal diffuse uterine hypertrophy, which is usually globular but rarely exceeds the size of a 12-week gestational age. The uterus swells during menstruation and may be tender during internal examination. It shrinks slightly and the tenderness disappears at the end of menstruation. Ultrasound examination can reveal that the entire uterus has become larger, the uterine wall has thickened, especially the posterior wall, and there is also an increase in ultrasound echoes. If the myometrium thickens and integrates into a tumor-like mass with strong echo characteristics, it is generally called a uterine adenomyoma. Recently, some people have used three-dimensional energy ultrasound to display changes in uterine myometrial blood flow for diagnosis. In addition, the tumor index CA-125 of patients with adenomyosis sometimes increases, which can be used as a reference for diagnosis. However, to confirm the diagnosis, it is still necessary to surgically remove the uterus or do a laparoscopic uterine biopsy and then send it for pathological examination. Age of onset and causes Adenomyosis often occurs in women aged 30 to 50, especially those approaching menopause. It is only discovered during a gynecological examination due to irregular menstruation and excessive menstrual bleeding. The true pathogenic mechanism is not clear, but some reports indicate that cesarean section scars and uterine scraping may also cause adenomyosis. When the enlarged uterus removed during surgery is opened and observed, fresh or old myometrial bleeding may be found. This is a manifestation of ectopic endometrial tissue in the myometrium. Some ectopic endometrial tissues in the myometrium may even undergo changes similar to the menstrual cycle, such as proliferation, secretion, and decidualization.
