The main manifestations of pelvic congestion syndrome are widespread chronic pain, extreme fatigue, and certain symptoms of neurasthenia. The most common symptoms are chronic lower abdominal pain, low back pain, unpleasant pleasure, extreme fatigue, excessive vaginal discharge and dysmenorrhea.

1. Lower abdominal pain

Most cases present with chronic diffuse pain above the pubic symphysis, or pain in the lower abdomen on both sides, which is often more severe on one side and also affects the same side or lower limbs at the same time, especially soreness and weakness in the groin or hip, which begins in the middle of the menstrual cycle. A small number of patients occasionally present with acute onset abdominal pain, which can be easily misdiagnosed as acute appendicitis, ruptured ovarian follicle, or ruptured ectopic pregnancy.

2. Low back pain

The pain is located at the level of the sacrogluteal region, and in a few cases in the lower half of the sacrum, often accompanied by lower abdominal pain. It is aggravated before menstruation, after standing for a long time and after sexual intercourse.

3. Dysmenorrhea

The characteristics are lower abdominal pain, lumbosacral pain or pelvic pain that begin several days before menstruation. Some cases gradually turn into spasmodic pain, which is most severe on the day before or on the first day of menstruation and is significantly relieved after the second day of menstruation.

4. Sexy is not happy

They often complain of varying degrees of pain during sexual intercourse, mostly deep pain, some of which are almost unbearable. The next day, symptoms such as lower abdominal pain, back pain, and excessive vaginal discharge are significantly aggravated, and the patients become bored with sexual life.

5. Extreme fatigue

6. Excessive vaginal discharge

Half of the patients had symptoms of excessive vaginal discharge. The characteristics of leucorrhea are mostly clear mucus and no signs of infection.

7. Menstrual changes

Some patients experience changes such as excessive menstruation, which are often misdiagnosed as uterine fibroids or uterine hypertrophy due to their enlarged uterus. Some patients also experience a decrease in menstrual volume compared to before. But it is accompanied by obvious premenstrual breast pain.

8. Congestion breast pain

Most patients have congestive breast pain and swelling. The patient herself can feel a lump and tenderness in the breast, which often appears after the middle of the menstrual period along with the above symptoms, reaching a peak on the day before menstruation or the first day of menstruation. After the menstruation, the above symptoms are alleviated or disappear completely. For some patients, breast pain is more severe than pelvic pain.

9. Swelling and pain in the vulva and vagina

Patients with pelvic congestion often experience swelling and heaviness in the vulva and vagina, or burning and itching in the vulva. The vulva may show pigmentation, swelling or hypertrophy of the labia majora, and some degree of venous engorgement, distension, or varicose veins.

10. Bladder and urethra symptoms

About one-third of patients have obvious symptoms of frequent urination and painful urination during the premenstrual period, but routine urine examination results are normal. Further cystoscopic examination of some patients with severe symptoms may reveal venous distension, congestion and edema in the trigone of the bladder. In some patients, rupture of small veins caused by congestion may lead to hematuria.

11. Rectal pain

Some patients have varying degrees of rectal heaviness, rectal pain or rectal pain during defecation, which is more obvious in the premenstrual period, especially in patients with grade III retroverted uterus.

12. Symptoms of the autonomic nervous system

The vast majority of patients with pelvic congestion syndrome have some symptoms of the autonomic nervous system. For example, the patient may feel irritable, easily excited, have many dreams at night, feel fatigued during the day, and have severe mental and physical incompetence. He or she may also often have headaches, which are mostly pain in the back of the head rather than the usual premenstrual headache type. There may be palpitations, discomfort in the precordial area, shortness of breath, frequent belching, abdominal distension, and difficulty passing gas. The patient feels poor appetite and indigestion. In addition, he experiences aches and pains all over the body, such as pain in the shoulder and hip joints, and tightness in the fingers.

13. Physical signs

The only finding of abdominal examination is tenderness, which is mostly located above the pubic symphysis or deep tenderness on both sides of the lower abdomen. Generally speaking, it is not significant, and there is not even a constant and particularly obvious tenderness point. Even in the part where the patient feels the most pain, there is no abdominal muscle tension or rebound pain. Some patients feel that there are lumps in their breasts, but examination only reveals diffusely swollen breast tissue below the nipple, which is often accompanied by varying degrees of tenderness.
