Many diabetic patients are obese, and the blood sugar-lowering effect and prognosis for this group of people are relatively poor.

Overweight and obesity are independent risk factors for type 2 diabetes. Among obese people, the incidence of type 2 diabetes increases year by year. More than 90% of diabetes patients in my country suffer from type 2 diabetes, and the incidence rate increases year by year. Patients with type 2 diabetes are often accompanied by systemic or abdominal obesity. Obesity leads to insulin resistance, which hinders pancreatic function and forms a vicious circle. Moreover, due to obesity, patients are insensitive to insulin, which leads to increased dosage of hypoglycemic drugs and even failure of sulfonylurea treatment.

Obesity is not only a chronic disease that endangers health, but also an important risk factor for a variety of chronic non-communicable diseases such as type 2 diabetes, cardiovascular and cerebrovascular diseases, and respiratory diseases. It is a global public health issue that affects the quality of life of patients.

Due to excessive energy intake, the adipose tissue of obese people reaches the maximum storage capacity, and the excess energy is released into the blood in the form of fatty acids and transported to other organs, resulting in an increase in free fatty acids (FFA) in the blood. When FFA transport to the liver increases, triglyceride accumulation in the liver increases, resulting in increased glucose output from the liver, reduced insulin uptake and insulin resistance. Increased levels of FFA and triglycerides in the circulation cause triglycerides to deposit in the muscles, impairing insulin signal transduction and metabolic enzyme activity.

High concentrations of FFA cause lipids to deposit in the pancreas, leading to reduced glucose-induced insulin release, impaired proinsulin synthesis, and increased B cell apoptosis. Eventually, various tissues and organs produce IR, which runs through the entire course of obesity-type 2 diabetes. Obesity-type 2 diabetes and IR are closely linked and inseparable. Under certain conditions, the two are mutually causal and mutually infiltrated, eventually forming a vicious cycle.

Therefore, diabetic patients with obesity must pay more attention. In addition to blood sugar-lowering treatment, weight loss treatment is also essential!
