Lumbar disc herniation is one of the more common diseases. The root cause is that the various parts of the lumbar disc (nucleus pulposus, annulus fibrosus and cartilage plate), especially the nucleus pulposus, have different degrees of degenerative changes. Under the action of external factors, the annulus fibrosus of the lumbar disc ruptures, and the nucleus pulposus tissue protrudes (or slips) from the ruptured part to the lateral back or inside the spinal canal, causing the adjacent spinal nerve roots to be irritated or compressed, thereby causing a series of clinical manifestations such as low back pain, numbness and pain in one or both lower limbs. The highest incidence of lumbar disc herniation is at L4-5 and L5-S1, accounting for about 95%.

1. Low back pain

It is the first symptom that occurs in most patients, with an incidence rate of about 91%. Because the longitudinal cruciate ligament behind the surface of the annulus fibrosus is irritated by the nucleus pulposus, the vertebral nerve causes induced pain in the lower back, sometimes accompanied by buttocks pain.

2. Radiating pain in the lower limbs

Although upper lumbar disc herniation (L2-3, L3-4) can cause femoral neuralgia, it is rare in clinical medicine, less than 5%. The vast majority of patients have prominent spaces between L4 and L5 and L5 and S1, and the main symptom is sciatica. Typical sciatica is radiating pain from the lower back to the buttocks, the back and sides of the thighs, the sides of the calves, and even the feet. The pain will be aggravated when the abdominal pressure increases, such as when sneezing and coughing. Radiating pain is mostly on one side of the body, and only very few patients with central or paracentral nucleus pulposus herniation mainly show lower limb symptoms. There are three causes of sciatica: the irritation of compounds caused by the ruptured lumbar disc and the body's own immune response cause the nerve roots to produce acid inflammation; the weight-bearing nucleus pulposus compresses or stretches the existing inflamed nerve roots, causing their venous return to be blocked, further aggravating edema and increasing sensitivity to pain; and ischemia of the nerve roots that were under full-term stress. The three elements are interrelated and reinforce each other.
