Symptoms of failed cerclage

The current treatment for cervical insufficiency is mainly vaginal cerclage after pregnancy, which is a procedure that sutures the vaginal end of the cervix in a circular manner to prevent the cervix from dilating and causing late miscarriage. The cerclage site of transvaginal cervical cerclage is in the vaginal part of the cervix and cannot reach the level of the internal os of the cervix. Therefore, the internal cervical os may still be open after cerclage during pregnancy, leading to uncontrollable uterine contractions and thus failure of cervical cerclage.

What is cervical cerclage?

Simply put, cervical cerclage is to use a silk thread to tie the cervix as close to the internal os of the cervix as possible, and then remove the silk thread at 37 weeks of pregnancy. Most women who experience multiple miscarriages may have cervical insufficiency, which causes the cervix to dilate and relax pathologically, leading to repeated miscarriages in the middle and late stages of pregnancy. Emergency cervical cerclage is the only way for pregnant women with cervical incompetence in mid-pregnancy to save the fetus. By suturing the cervix, the fetus can continue to develop and grow in the mother's belly.

When is cervical cerclage necessary?

1. Very few expectant mothers will encounter such a situation: 15-26 weeks of pregnancy, most of which are around 20 weeks, without any discomfort symptoms. Some people feel that the vaginal discharge is more than usual, or feel a little discomfort in the lower abdomen. During the prenatal check-up, the B-ultrasound shows that the cervix is ​​relaxed. When the cervix has dilated and the fetal membrane has bulged out of the cervix, in order to prevent the occurrence of spontaneous abortion and save the normally developed but immature fetus, timely implementation of emergency cervical cerclage may save the life of the fetus.

2. If the patient has had a late miscarriage or premature birth in the past, he or she must go to a regular hospital to check the tightness of the cervix before getting pregnant again. If diagnosed with "cervical insufficiency", it is best to perform a preventive cervical cerclage at 14-18 weeks of pregnancy to prevent miscarriage and premature birth, and achieve "early detection, early diagnosis, and early intervention" to ensure the smooth full-term birth of the baby.
