What to do if you have ectopic pregnancy or ovarian pregnancy

Ectopic pregnancy and ovarian pregnancy are a common type of ectopic pregnancy among women. It is mostly caused by inflammation in the pelvic cavity. It will not only cause massive bleeding in the female abdominal cavity, but also cause shock in severe cases. Therefore, for the condition of ectopic pregnancy and ovarian pregnancy, female friends must make sure to detect and treat it early. So what should women do after they have an ectopic pregnancy or ovarian pregnancy?

The treatment of ovarian pregnancy is mainly surgical. During surgery, ovarian tissue should be preserved as much as possible. Depending on the size of the lesion, ovarian pregnancy lesion resection, ovarian wedge lesion resection or partial oophorectomy can be performed. If the bleeding is not severe and the lesion is found to involve only part of the ovary during laparotomy, a wedge resection of the affected area can be performed. After the remaining part of the ovary is sutured, the blood β-HCG level is monitored postoperatively. If it decreases slowly, it means that some villous tissue may still remain, which can be treated with methotrexate.

If the ovary is severely damaged, an oophorectomy may be performed on the affected side. It is generally not recommended to simply remove the ovary while retaining the ipsilateral fallopian tube, as this may cause the fertilized egg to migrate outside the body and increase the chance of tubal pregnancy. Surgery can also be performed laparoscopically.

If laparoscopy confirms an unruptured ovarian pregnancy, conservative treatment can be performed by injecting methotrexate directly into the ovarian gestational sac. If an ovarian pregnancy has been diagnosed and is not ruptured before surgery, methotrexate can be injected into the gestational sac under B-ultrasound guidance, and systemic treatment with methotrexate can also be performed. During treatment, blood β-HCG is used for monitoring. If rupture or excessive internal bleeding occurs, surgical treatment is still required.
