In severe cases, there is obvious rebound tenderness in the lower abdomen, but the abdominal muscle rigidity is not as strong as in generalized peritonitis. 3.2 Bimanual examination There is tenderness in the fornix on both sides of the cervix when the cervix is lifted. The uterus is of normal size. When the uterus is painful to move and there is heavy internal bleeding, the adnexal area or posterior fornix may be felt to be swollen.
