What to do if pregnancy is complicated by uterine fibroids

If pregnancy is complicated by uterine fibroids, it is necessary to pay attention to whether the situation is serious or whether it is necessary to terminate the pregnancy. After all, if these diseases are serious or have already affected the fetus, then it is necessary to pay attention to taking effective treatment methods.

1. Pre-pregnancy consultation and treatment

If the uterine fibroids are non-submucosal and are 4 cm in diameter, they are prone to induce degeneration and miscarriage (premature birth) during pregnancy. It is recommended to undergo a uterine fibroid removal before pregnancy and then become pregnant.

2. Pregnancy monitoring

Regular B-ultrasound check-ups during pregnancy should be performed to understand changes in the fibroids. Generally, fibroids will increase significantly during pregnancy. If there are no symptoms, no treatment is needed.

3. Treatment of uterine fibroid degeneration during pregnancy

If the patient's fibroids degenerate during pregnancy, causing uterine contractions, hospitalization is required and antibiotics can be given. If the uterine contractions are obvious, drugs that inhibit uterine contractions can be given.

4. Childbirth

If the fibroids do not block the birth canal, vaginal delivery is possible. However, uterine contractions and progress should be monitored during labor. If uncoordinated contractions or weak contractions occur and treatment is ineffective, a cesarean section may be required. Be alert to placental adhesion after delivery and postpartum hemorrhage caused by poor uterine contractions.

5. Cesarean section

It is suitable for patients whose birth canal is blocked by uterine fibroids or who have other obstetric indications for cesarean section. The size of uterine fibroids can be detected during the operation. Because fibroids are rich in blood vessels and have unclear boundaries during pregnancy, they can easily cause heavy bleeding during peeling. Therefore, it is not recommended to perform myomectomy during cesarean section, unless it is a subserosal fibroid or a pedunculated fibroid, which can be removed during cesarean section because of its low surgical risk.
