Sometimes patients experience stiffness in the waist muscles when they get up in the morning, which can be relieved after exercise; sometimes they experience heel pain and other inflammation of the tendons and tendon bone attachment points. About more than half of the patients may also experience inflammation of different joints and muscle soreness. The subsequent remission period is often without any clinical symptoms and sometimes lasts for many years, especially in female patients.

The early symptoms of spinal disease are relatively hidden, and may include systemic symptoms such as anorexia, low-grade fever, fatigue, weight loss and anemia. A very small number of cases may have long-term low-grade fever and joint pain, which are similar to the manifestations of rheumatic fever. Inflammatory manifestations such as high fever and jugular joints are not uncommon. This type of case is more common in younger people, and is often accompanied by symptoms such as weight loss and night sweats. Some symptoms often present as lumbar pain, often with dull pain, and sometimes occur after fatigue or injury;

It is often accompanied by rotational deformity of the spine and changes in the physiological curvature of the sagittal plane.
