Pelvic floor muscle exercises

The method introduced here is very simple. It does not require any equipment and can be performed at any occasion and time without being noticed by others. The method is as follows: Contract the vagina and anus, lift up, hold for 2 to 3 seconds, relax, and then repeat the previous action. This exercise should be done 300 to 500 times a day. These movements do not have to be completed in one go, but can be completed in several times. Generally, they will take at least 1 to 2 months to be effective, and need to be continued for more than a year. It may be effective for mild stress urinary incontinence.

Drug treatment

Adrenergic receptor agonists (such as guantong and ephedrine) are the most commonly used drugs for the treatment of stress urinary incontinence and are also considered to be the most effective drugs in current research. This type of drug can treat stress urinary incontinence by increasing the closure ability of the urethral sphincter. Some cases can be treated with anticholinergic drugs, such as propantheline bromide, imipramine, and oxybutynin hydrochloride. Estrogen can be used to treat stress urinary incontinence in postmenopausal women. Estrogen can increase the tension and blood supply of the urethral sphincter, but its efficacy is still controversial. In addition, patients with stress urinary incontinence who have breast cancer, cervical cancer, or uterine cancer should not receive estrogen medication.

Surgery

Surgical treatment methods include: posterior urethral injection of sclerosing agents, various suspension techniques, artificial urinary sphincter insertion, and urethral lengthening or plication. Among them, the most mature treatment method internationally is the tension-free vaginal tape implantation (TVT implantation). The operation involves inserting a sling made of special polypropylene material into the body. When the intra-abdominal pressure increases, the sling can be lifted relatively to compress the urethra, thereby inhibiting urine leakage. This sling does not cause inflammation in the body and can be effective for life. The operation is minimally invasive with minimal damage, generally not damaging the bladder, urethra, intestines, uterine appendages, etc.; and the puncture point is concealed and will not affect the patient's aesthetic requirements.
