On July 21, a 41-year-old male patient was rescued for a sudden large-area myocardial infarction. Although the blood vessels were opened at that time, the patient continued to suffer from hypotension and shock after the operation. Almost all available rescue drugs were used, a ventilator was used, an aortic balloon device was used, and he stayed in the cardiac intensive care unit for 7 days, but he finally died of heart failure...

At first, the family members kept saying that the patient was fine, so how could he suddenly have a heart attack?

The patient is about 1.76 meters tall and weighs 180 kilograms. Without calculating the BMI, he is definitely seriously overweight, which is a risk factor - obesity.

The family members did say that the child is just a little fat, doesn’t like to be active, and likes to eat delicious food, but he doesn’t have any disease!

After admission, the random blood sugar test showed 18, fasting blood sugar 11, triglycerides 6.3, low-density lipoprotein 5.4, high-density lipoprotein 0.3; this is a very typical case of diabetes and hyperlipidemia.

As to whether the patient originally had high blood pressure, it is currently unknown because the patient suffered from low blood pressure after the myocardial infarction. However, according to family members' recollections, the patient had measured his blood pressure once, which was around 170, but he did not feel uncomfortable, so he did not care.

Considering these facts, is it true that the patient was fine all of a sudden and had a heart attack as the family members said? Of course not!

Patient: Obesity + hypertension + diabetes + hyperlipidemia = metabolic syndrome.

1. The harm of metabolic syndrome is huge:

Metabolic syndrome is a group of complex metabolic disorders. It is a risk factor for diabetic cardiovascular and cerebrovascular diseases and can cause an increase in a variety of diseases, such as hypertension, coronary heart disease, stroke, and even certain cancers.

Compared with patients without metabolic syndrome, patients with metabolic syndrome have a 1.85-fold increased risk of cardiovascular disease, and a 2.41-fold and 1.63-fold increased risk of ischemic and hemorrhagic stroke, respectively.

Metabolic syndrome types:

Obesity + hypertension + hyperlipidemia = cardiovascular risk increased 5.25 times;

Obesity + hypertension + hyperlipidemia + diabetes = 16.58 times increased risk of stroke.

2. How to identify metabolic syndrome

Simply put, metabolic syndrome can be diagnosed if three of the four symptoms are met: obesity, hypertension, diabetes, and hyperlipidemia.

1. Obesity: waist circumference ≥90cm for men and ≥85cm for women;

2. Hypertension: blood pressure ≥130/85mmHg and/or those who have been diagnosed with hypertension and received treatment;

3. Hyperlipidemia: fasting triglyceride ≥ 1.7 mmol/L, fasting high-density lipoprotein < 1.04 mmol/L, or confirmed dyslipidemia and treated with medication;

4. Diabetes: fasting blood glucose ≥6.1mmol/L or blood glucose ≥7.8mmol/L 2 hours after glucose load, and/or those who have been diagnosed with diabetes and are receiving treatment.

3. Young people, especially middle-aged people, should pay more attention to the control of metabolic syndrome

Metabolic syndrome is gradually increasing in my country. In 2002, the rate of metabolic syndrome among people over 18 years old in my country was 13.8%, and it increased to 18.2% in 2009. Among them, hypertension is the most common, accounting for 65.4%, followed by dyslipidemia: 53.6% of men have hyperlipidemia and 49.4% of women have it.

Our lives, especially eating habits, lack of exercise, nature of work, changes in the pace of life, have caused more people to become fatter, have metabolic disorders, and eventually develop metabolic syndrome.

If metabolic syndrome is found, it should be actively controlled. If metabolic syndrome does not occur, it should also be actively prevented. Only a healthy life is the basis for preventing and controlling metabolic syndrome. Eat a low-oil, low-salt, low-sugar diet, reduce the proportion of refined grains, increase the proportion of coarse grains, vegetables and fruits; reduce the proportion of fatty meat, animal offal, and red meat, and focus on chicken, fish and other meats. Add some nuts, dairy products, etc.

Adhering to proper exercise can reduce the risk of metabolic syndrome by 10% to 20%. At the same time, quit smoking and drinking, stay up late less, and maintain a good mood. These are all conducive to controlling weight and preventing and controlling the three highs.

4. However, once metabolic syndrome occurs or develops, it must be actively corrected and controlled:

Antihypertensive drugs: Pril or sartan are recommended as the first choice, especially for patients with diabetes or obesity; dihydrotestosterone can also be used; for patients with heart failure and coronary heart disease, diuretics and loras can be used.

Lipid-lowering drugs: Statins are generally the first choice for elevated low-density lipoprotein and cholesterol, which not only lower low-density lipoprotein but also increase high-density lipoprotein; fibrates are generally used for elevated triglycerides.

Hypoglycemic drugs: Choose hypoglycemic drugs under the guidance of a doctor based on your personal age and weight.

Let’s take a look at this 41-year-old patient. His family said he was fine. How did he suddenly suffer a myocardial infarction?

Obesity, inactivity, uncontrolled diet, diabetes, hyperlipidemia, and hypertension; this is not only metabolic syndrome, but also metabolic syndrome+.
