Pelvic congestion syndrome, also known as ovarian vein syndrome, is one of the important causes of gynecological pelvic pain. The main manifestations of pelvic congestion syndrome are widespread chronic pain, extreme fatigue and certain symptoms of neurasthenia. The most common symptoms are chronic lower abdominal pain, low back pain, unpleasant pleasure, extreme fatigue, excessive vaginal discharge and dysmenorrhea. Because its symptoms are extensive, and the patient's subjective symptoms are often inconsistent with objective examinations, the physical signs are often confused with chronic pelvic inflammatory disease.

Causes

Any factor that causes pelvic venous outflow from the pelvis to be obstructed or blocked can cause pelvic venous congestion.

1. Anatomical factors

The characteristics of female pelvic circulation are mainly an increased number of veins and weak structure.

2. Internal and external factors

Internal and external forces have been shown to affect the flow rate of pelvic blood, thereby changing the pressure of local blood vessels, and veins are more susceptible to their influence.

(1) People who work in standing or sitting positions for a long time or are accustomed to sleeping in a supine position are more likely to develop pelvic congestion syndrome.

(2) Constipation affects the venous return of the rectum, and the rectum, uterus, and vaginal veins are anastomosed with each other. Congestion of the hemorrhoidal plexus will inevitably cause congestion of the uterine and vaginal plexus, so habitual constipation can easily cause pelvic congestion.

(3) During pregnancy, the effects of large amounts of androgens and progesterone, coupled with the compression of the enlarged uterus on the veins around the uterus, can cause dilation of the veins around the uterus.

(4) When the uterus is retroverted, the ovarian plexus blood vessels bend along the sides of the sacral concave region as the uterus descends, increasing venous pressure and affecting venous return, causing the veins to become congested. If you have the habit of sleeping in the supine position, it may lead to pelvic congestion syndrome over time.

(5) Tears of the broad ligament fascia cause the veins, which are structurally weak and lack elasticity and lack an intrinsic vascular sheath, to lose support, forming varicose veins and causing the uterus to fall back.

(6) Tubal ligation: The specific causes of the disease are not clear, and there is no clear evidence to prove the causal relationship with the surgery.
