Side effects of penicillin

1. Allergic reaction. This type of side effect is generally the most common. Skin allergies (urticaria, drug rash, etc.) and serum sickness-like reactions are more common, but they are generally not serious and will disappear after stopping the drug. The most serious of these adverse reactions is anaphylactic shock, the main clinical manifestations of which are circulatory failure, respiratory failure and central nervous system depression. To prevent the occurrence of such side effects, skin allergy tests must be conducted when using drugs such as penicillins (including penicillin), penicillamine and penicillin derivatives. If the test results are positive, the above-mentioned drugs are prohibited.

2. Herxheimer reaction. When penicillin is used to treat diseases such as syphilis, leptospirosis, yaws, rat-bite fever, anthrax, etc., symptoms may worsen, manifested as general discomfort, chills, fever, sore throat, myalgia, rapid heartbeat, etc. This reaction may be caused by substances (endotoxins, foreign proteins, etc.) released after the pathogens are killed in large quantities by penicillin.

3. Others. Intramuscular injection of penicillin may produce local pain, swelling, and induration. Excessive doses or rapid intravenous administration may cause direct stimulation to the cerebral cortex. Intrathecal injection may cause symptoms of meningeal and nerve irritation. Intravenous infusion of large doses of penicillin potassium or sodium salt can cause significant water and electrolyte imbalance, especially in patients with decreased renal function, it can cause hyperkalemia and hypernatremia, and even cardiac function inhibition. To be honest, when the last adverse reaction occurs, it is sometimes no different from a death sentence by injection. In addition, when penicillin is given intravenously, sodium salt is usually used, and potassium salt is basically not used.
