Primary vaginal malignancies are rare, accounting for approximately 1% of malignancies of the female reproductive organs. The main types are squamous cell carcinoma and chloasma carcinoma. Others such as adenocarcinoma, sarcoma and malignant melanoma are even rarer. Many obstetricians and gynecologists have only seen a few patients in their medical practice. Since secondary cancer of the vagina is more common, the possibility of secondary vaginal cancer should be considered and excluded before diagnosing the primary tumor.

Involvement of the urethra or bladder may cause frequent urination, urgency, hematuria, and dysuria; involvement of the rectum may cause difficulty in defecation and tenesmus; when the paravaginal, cardinal ligament, and uterosacral ligament are invaded, lumbar pain may occur. If it is a vaginal leiomyoma, the most common symptom is vaginal and rectal pain, which occurs in about half of the cases.

Western medicine treatment of vaginal malignant tumors

Treatment principles: Vaginal cancer can be treated with surgery or radiotherapy. The treatment of cancer in the upper part of the vagina is the same as that of cervical cancer, the treatment of cancer in the lower part is the same as that of vulvar cancer, and the treatment of cancer in the middle part requires consideration of both. If the bladder or rectum is invaded, organ removal and diversion are required.

Surgical treatment: ① Radical hysterectomy, partial colpostomy and pelvic lymphadenectomy, suitable for early cancer of the upper vagina. ② Radical resection of the vulva and vagina and inguinal or pelvic lymphadenectomy are suitable for smaller and localized lesions in the lower vagina. ③ The above ① or ② plus organ removal and diversion. This type of surgery is very extensive and carries a high risk of surgical trauma and complications, so it should not be used unless absolutely necessary.

Chemotherapy (radiotherapy): The radiotherapy plan depends on the location and extent of the cancer. The treatment of tumors in the fornix is ​​the same as that for cervical cancer. If there is infiltration around the vagina, the whole pelvis is irradiated followed by local radium therapy. The five-year survival rate for vaginal cancer is generally 35%. The cause of death is often uremia or infection caused by obstruction of the urinary system.
